About Xarelto tablet 15mg
Xarelto Tablet 15mg contains Rivaroxaban, a factor Xa inhibitor used for initial DVT or PE treatment and AFib dosing.
Xarelto Tablet 15mg Uses
- Treating deep vein thrombosis (DVT) during the initial three week high dose phase
- Treating pulmonary embolism (PE) during the initial three week high dose phase
- Preventing stroke and blood clots in non valvular atrial fibrillation patients with reduced kidney function
- Reducing clot formation risk in patients who cannot take the standard 20mg AFib dose
Xarelto Tablet 15mg Benefits
- Works as a single oral tablet without the need for regular blood monitoring
- Allows a smooth switch from twice daily dosing to once daily maintenance therapy
- Gives kidney impaired AFib patients a safer, dose adjusted anticoagulation option
- Reduces hospital stay by allowing early oral treatment of confirmed DVT or PE
What Experts Say
Physicians in Pakistan prescribe Xarelto Tablet 15mg mainly during the first three weeks of DVT or PE treatment, when clot formation risk is highest and a stronger dose is needed to stabilize the patient.
This twice daily regimen achieves faster therapeutic drug levels than the standard maintenance dose. Doctors then transition patients to the 20mg once daily tablet once the acute phase is complete. In atrial fibrillation, cardiologists reserve the 15mg strength for patients whose kidney function falls in the moderate impairment range, since reduced renal clearance raises bleeding risk at the full 20mg dose. This dose adjustment protects against drug accumulation while still lowering stroke risk effectively.
Pakistan has a high burden of undiagnosed atrial fibrillation and late presenting venous thromboembolism, making accurate dosing especially important. Experts stress that patients should never skip the initial higher dose phase, since under dosing early in DVT or PE treatment raises the chance of clot extension or recurrence.

Drug Interactions
- Ketoconazole and itraconazole: These antifungals raise rivaroxaban blood levels and increase bleeding risk
- Ritonavir: This antiviral significantly increases rivaroxaban exposure
- Rifampicin: This antibiotic lowers rivaroxaban levels and can reduce its clot preventing effect
- NSAIDs like ibuprofen and aspirin: Combined use raises the risk of gastrointestinal bleeding
- Other anticoagulants like warfarin or heparin: Combining these increases bleeding risk substantially
- Selective serotonin reuptake inhibitors (SSRIs): These antidepressants may increase bleeding tendency when combined with rivaroxaban
Risks and warnings
- Bleeding risk: Rivaroxaban can cause serious or life threatening bleeding, especially during the higher dose initial treatment phase
- Spinal or epidural hematoma: Patients undergoing spinal puncture or epidural anesthesia while on this medicine face a risk of spinal bleeding that can cause long term paralysis
- Renal impairment: Patients with severe kidney disease require dose adjustment or should avoid this medicine, since reduced clearance increases drug levels
- Premature discontinuation: Stopping this medicine early without switching to another anticoagulant raises the risk of stroke or clot recurrence
- Liver disease: Patients with significant liver impairment linked to bleeding risk should avoid this medicine
- Pregnancy: This medicine is not recommended during pregnancy due to bleeding risk to the mother and fetus
