What Experts Say
Gravinate contains Dimenhydrinate, a combination of diphenhydramine and 8-chlorotheophylline. It belongs to the ethanolamine class of antihistamines and works by blocking H1 histamine receptors in the brain and inner ear. By suppressing stimulation of the vomiting center and reducing sensitivity of the vestibular apparatus, it effectively controls nausea, vomiting, and dizziness.
Dimenhydrinate is well established in clinical practice for motion sickness prophylaxis. When taken 30 minutes before travel, it significantly reduces the likelihood of nausea and vomiting during the journey. Its central nervous system depressant effect also contributes to its antiemetic action, though this same property is responsible for its sedating side effect.
Clinicians also use Dimenhydrinate for vestibular conditions such as Meniere's disease and labyrinthitis, where inner ear disturbance causes persistent dizziness and nausea. It is one of the more commonly available antiemetics in Pakistan and is frequently used in both outpatient and emergency care settings.

Drug Interactions
Patients should inform their doctor about all medicines they are currently taking before starting Gravinate. Key interactions include:
- Alcohol: increases sedation and central nervous system depression. Avoid during treatment
- Other antihistamines: combining with additional antihistamines raises the risk of excessive sedation and anticholinergic effects
- Sedatives, sleeping tablets, and benzodiazepines: concurrent use significantly increases drowsiness and impairs coordination
- MAO inhibitors such as phenelzine and tranylcypromine: may intensify and prolong the anticholinergic effects of Dimenhydrinate
- Aminoglycoside antibiotics such as gentamicin: Dimenhydrinate may mask signs of ototoxicity caused by these drugs
- Anticholinergic medicines: additive effects may increase the risk of dry mouth, urinary retention, and constipation
Risks and warnings
- Sedation: Gravinate causes significant drowsiness. Patients must not drive or operate machinery until they know how the medicine affects them.
- Children: Dimenhydrinate can cause paradoxical excitability or restlessness in some children. Use with caution in pediatric patients.
- Elderly patients: Older adults are more sensitive to anticholinergic effects including confusion, urinary retention, and falls. Use the lowest effective dose.
- Glaucoma: Gravinate may worsen angle-closure glaucoma. Patients with this condition must not use it without specialist advice.
- Enlarged prostate: Anticholinergic effects can worsen urinary retention in patients with benign prostatic hyperplasia.
- Asthma and respiratory conditions: Use with caution in patients with chronic respiratory disease as Dimenhydrinate may thicken bronchial secretions.
- Pregnancy: Gravinate is not recommended during pregnancy unless specifically advised by a doctor. All risks and benefits must be discussed first.
- Breastfeeding: Dimenhydrinate passes into breast milk and may cause drowsiness in the infant. A doctor must be consulted before use.
