What Experts Say
Aripiprazole acts as a partial agonist at dopamine D2 receptors rather than a full blocker, which sets it apart from older antipsychotics. This partial activity calms overactive dopamine signaling linked to hallucinations and delusions, while avoiding the complete blockade that causes severe movement side effects with many older drugs in this class.
Psychiatrists often favor this molecule for patients who struggled with weight gain, sedation or high prolactin on other antipsychotics, since its metabolic profile tends to be milder overall. Doctors still check weight, blood sugar and cholesterol periodically during long term treatment, since some patients do gain weight or develop glucose changes regardless.
A boxed warning applies to elderly patients with dementia related psychosis, who face a higher risk of death when treated with this drug class, so aripiprazole is not approved for that use. Doctors also watch younger patients closely in the early weeks of treatment for any worsening mood or new suicidal thoughts, a precaution that applies broadly across this class of medicine.
