What Experts Say
Doctors in Pakistan increasingly prescribe Adenuric Tablet 80mg for chronic gout when allopurinol, the older first line urate lowering drug, is not tolerated or fails to bring uric acid to target. Rising red meat and seafood consumption, obesity and chronic kidney disease have pushed gout rates higher across the country.
International drug regulators, including the FDA, added a boxed warning to febuxostat after a large cardiovascular outcomes trial linked it to a higher rate of heart related deaths compared with allopurinol. Physicians now reserve this tablet mainly for patients who have already tried allopurinol, and they screen for existing heart disease before starting treatment and monitor closely afterward.
Starting febuxostat can trigger a temporary gout flare as stored uric acid crystals are mobilized from the joints, so doctors commonly add a low dose NSAID or colchicine for the first few months. Liver function tests are checked before treatment starts and periodically during therapy since the drug is processed by the liver.

- Colchicine: Commonly combined during the first months of treatment to prevent gout flares under medical supervision
Risks and warnings
- Cardiovascular Risk: Febuxostat carries a boxed warning for a higher risk of heart related death compared with allopurinol, so it is generally reserved for patients who cannot use allopurinol
- Gout Flare At Start: Lowering uric acid can trigger a temporary increase in gout attacks during the first few months of treatment
- Liver Problems: Can raise liver enzymes, so liver function should be checked before and during treatment
- Heart Disease History: Use with caution in patients with a history of heart attack, stroke or heart failure
- Kidney Impairment: Dose adjustment may be needed in patients with significant kidney disease
