How It Works
Betamethasone Valerate is a potent topical corticosteroid that suppresses the inflammatory cascade in the skin. It binds to intracellular glucocorticoid receptors, triggering genomic effects that reduce the production of pro-inflammatory cytokines, inhibit phospholipase A2, decrease vascular permeability, and suppress immune cell activity in the dermis. The result is rapid reduction in redness, swelling, itching, and heat at the site of inflammation. Betamethasone Valerate at 0.1% is classified as a potent corticosteroid under the UK topical steroid potency classification, significantly stronger than hydrocortisone.
The Role of Neomycin
Neomycin Sulfate is an aminoglycoside antibiotic that works by binding irreversibly to the 30S ribosomal subunit of bacterial cells, causing misreading of mRNA during protein synthesis and ultimately disrupting bacterial cell membrane integrity, leading to cell death. When applied topically, Neomycin is active against a broad range of gram-positive and gram-negative bacteria including Staphylococcus aureus, the most common secondary bacterial pathogen in eczema and dermatitis. However, Neomycin has no activity against Pseudomonas aeruginosa or anaerobes.
Clinical Note
Neomycin is one of the most common causes of contact allergy in Pakistan and globally. Sensitization rates are significant, and a patient who develops a new rash or worsening irritation while using Betnovate N should be assessed for Neomycin contact allergy rather than assuming treatment failure. Betnovate N should never be used as a skin lightening or cosmetic product, a practice that is widespread in Pakistan and causes severe and irreversible skin damage. Prolonged use on the face, skin folds, or genitals can cause cutaneous atrophy, striae, and systemic absorption leading to adrenal suppression.

When Not to Use Betnovate N
- Known allergy or hypersensitivity to Betamethasone, Neomycin, or any other aminoglycoside antibiotic.
- Viral skin infections including herpes simplex, chickenpox, and shingles. Corticosteroids suppress immune responses and can dramatically worsen viral infections.
- Fungal skin infections such as ringworm, athlete's foot, and candidiasis. Corticosteroids can allow fungi to proliferate unchecked.
- Rosacea, acne vulgaris, and perioral dermatitis, as corticosteroids worsen all three conditions.
- Skin tuberculosis.
- Perianal and genital pruritus without a confirmed diagnosis.
- Children under 1 year of age, and with caution in children under 12.

- Betamethasone Valerate + Neomycin
How to use Betnovate N Cream 20g?
- Clean and gently dry the affected skin area before applying the cream.
- Apply a thin layer to the affected area two to three times daily or as directed by the prescribing doctor.
- Rub in gently until the cream is absorbed. Do not apply excessive amounts.
- Wash hands thoroughly after application unless the hands are the area being treated.
- Do not apply to the face unless specifically directed by a doctor. If used on the face, limit use to 5 to 7 days maximum.
- Do not cover the treated area with an airtight dressing or bandage unless instructed by a doctor, as occlusion dramatically increases absorption and side effect risk.
- The standard treatment duration is 7 to 14 days. Treatment should not be continued beyond 2 weeks without medical review.
- The cream should be stopped gradually under medical supervision rather than abruptly after prolonged use, to avoid topical steroid withdrawal.
Drug Interactions
Betnovate N is applied topically and has limited systemic drug interactions at standard doses. However, the following should be noted:
- Other topical corticosteroids: Using two topical corticosteroids on the same area simultaneously increases the risk of skin atrophy, systemic absorption, and adrenal suppression.
- Other aminoglycoside antibiotics (systemic or topical): Concurrent systemic aminoglycoside use alongside Neomycin-containing creams may increase the risk of ototoxicity and nephrotoxicity in patients with large affected areas or compromised skin barriers.
- Immunosuppressants such as cyclosporine: Topical corticosteroids may have an additive immunosuppressive effect, increasing infection risk.
Risks and warnings
- Not for skin lightening: Betnovate N is widely misused in Pakistan as a skin lightening or fairness cream. This practice causes irreversible skin damage including atrophy, striae, telangiectasia, and permanently compromised skin barrier function. It is not safe or appropriate for cosmetic use.
- Face and skin fold caution: Absorption through facial skin, skin folds, and genital skin is significantly higher than through thick body skin. Use on these areas must be limited to short courses under strict medical supervision.
- Viral and fungal infections: Betnovate N must never be applied to skin infected with viruses (herpes, chickenpox) or fungi (ringworm, candida). The corticosteroid component suppresses the immune response, causing these infections to spread rapidly and severely.
- Neomycin contact allergy: Neomycin is a common sensitizer. Any worsening of the skin condition while using Betnovate N should prompt evaluation for Neomycin-induced contact dermatitis, which requires stopping the cream and avoiding all aminoglycoside-containing products.
- Topical steroid withdrawal (TSW): Prolonged use of potent topical steroids can cause physical dependence of the skin. Stopping abruptly after long-term use may trigger severe rebound inflammation. The cream must be tapered gradually under medical guidance.
- Children: The skin of children absorbs topical corticosteroids more readily than adult skin, increasing the risk of systemic side effects. Betnovate N should be used in children only under medical supervision, for the shortest necessary period, and never under a diaper.
- Pregnancy: Topical corticosteroids are absorbed through the skin. They should only be used during pregnancy when the benefit clearly outweighs the risk, and only on small areas for short periods.
- Breastfeeding: Betnovate N should not be applied to the breast area during breastfeeding.
- Ear use: Do not use in the ear if there is any suspicion of a perforated eardrum.
Betnovate N Cream Side Effects
Most patients tolerate Betnovate N well when used correctly for short treatment periods. Side effects are more common with prolonged use, large area application, occlusive dressings, and use on the face or skin folds.
- Skin irritation, burning, or stinging at the application site, particularly in the first few days.
- Dryness or peeling of the treated skin.
- Contact allergy to Neomycin presenting as worsening redness, itching, or a new rash during treatment.
- Skin thinning (cutaneous atrophy) with prolonged use, particularly on the face and skin folds.
- Stretch marks (striae) with prolonged use on skin folds or under occlusion.
- Perioral dermatitis or acne-like rash if used on the face.
Serious side effects with prolonged or widespread use require medical attention:
- Adrenal suppression caused by systemic absorption of Betamethasone, presenting as fatigue, weight loss, and reduced stress response.
- Cushing's syndrome features including central obesity, moon face, and easy bruising with extensive long-term use.
- Topical steroid withdrawal presenting as severe rebound redness, burning, and inflammation when the cream is stopped after prolonged use.
- Neomycin sensitization causing persistent contact allergy that affects future use of any aminoglycoside-containing product.
