What Experts Say
Atenolol is a beta blocker that works by blocking beta-1 receptors found mainly in the heart. This action slows the heart rate, reduces the force of each heartbeat and lowers the oxygen the heart muscle needs. Because atenolol is cardioselective, it has less effect on the airways than older beta blockers, though caution is still needed in people with asthma.
Large clinical trials have shown atenolol effectively lowers blood pressure and reduces angina episodes. Outcome data in patients after a heart attack shows a reduced risk of further cardiac events when atenolol is started early and continued as advised. Its once daily dosing makes it a practical choice for long term blood pressure control in routine practice.
Doctors typically start with a lower dose and adjust it based on blood pressure readings, heart rate and how well the medicine is tolerated. Sudden discontinuation is avoided because it can trigger rebound high blood pressure or worsen angina. Regular monitoring helps balance effective control with minimal side effects for each patient.

Drug Interactions
- Calcium channel blockers: Combining with drugs such as verapamil or diltiazem can cause a dangerously slow heart rate or low blood pressure.
- Other antihypertensives: Using them together can add to the blood pressure lowering effect and cause dizziness or fainting.
- Insulin or diabetes tablets: Atenolol can hide the usual symptoms of low blood sugar, so monitoring is essential.
- NSAIDs: Regular use of drugs such as ibuprofen or diclofenac can reduce atenolol's blood pressure lowering effect.
- Clonidine: Stopping clonidine while still on atenolol can cause a dangerous rise in blood pressure.
- Digoxin: Combined use raises the risk of a very slow heart rate and heart block.
Risks and warnings
- Sudden discontinuation: Stopping abruptly can worsen angina and increase the risk of an irregular heartbeat or heart attack.
- Asthma or COPD: Atenolol can narrow the airways and worsen breathing difficulty in people with lung disease.
- Diabetes: It can mask warning signs of low blood sugar such as a fast heartbeat, so glucose should be monitored closely.
- Heart block or slow heart rate: Not recommended in patients with significant heart block or a very slow pulse.
- Pregnancy and breastfeeding: Should be used only if clearly needed, since it may affect fetal growth and passes into breast milk.
- Kidney impairment: Dose adjustment is needed because atenolol is cleared mainly through the kidneys.
- Before surgery: Inform your doctor and anesthetist that you are taking this medicine, since it affects the heart's response during surgery.
