What Experts Say
Alendronate works by attaching to bone surfaces and slowing down osteoclasts, the cells that break down old bone. This shifts the balance toward bone formation, gradually increasing bone density over months of consistent use.
It needs to be taken on an empty stomach with plain water, since food and other drinks reduce its absorption drastically. Staying upright for at least 30 minutes after taking it also helps prevent irritation to the food pipe, which is one of the more common complaints with this drug class.
Doctors usually reserve alendronate for patients with confirmed low bone density on a DEXA scan or a history of fragility fractures, since long-term bisphosphonate use needs periodic reassessment of ongoing benefit versus risk, particularly after several years of continuous treatment. Adequate calcium and vitamin D intake alongside treatment supports the bone-building effect.
