What Experts Say
Escitalopram works by blocking the reuptake of serotonin in the brain, which increases its availability at nerve synapses. This gradual increase in serotonin activity helps regulate mood, emotional response and anxiety levels over time. Full therapeutic benefit usually appears after several weeks of consistent use, not immediately.
The 10mg strength is often the starting dose for most adults beginning escitalopram therapy. A lower initial dose allows the body to adjust to the medication while reducing the chance of early side effects such as nausea or restlessness. Many patients respond well at this dose alone, without needing an increase to the 20mg strength.
Clinicians typically reassess response after four to six weeks of steady use, since SSRIs need this window to show their full effect. Escitalopram is considered one of the more selective and better tolerated SSRIs available, with a relatively low interaction burden compared to older antidepressants. Sudden discontinuation is generally avoided, since the dose is usually tapered under medical supervision.

Drug Interactions
- MAOIs: Combining escitalopram with monoamine oxidase inhibitors is contraindicated due to a high risk of serotonin syndrome, and a washout period of at least two weeks is required between the two.
- Other SSRIs or SNRIs: Taking escitalopram with another SSRI or SNRI raises serotonin levels further and increases the risk of serotonin syndrome.
- Tramadol: This combination raises serotonin levels and can also lower the seizure threshold.
- Triptans: Migraine medicines in this class can add to serotonergic activity, increasing the chance of serotonin syndrome.
- NSAIDs: Regular use with painkillers like ibuprofen or aspirin increases the risk of abnormal bleeding.
Risks and warnings
- Suicidal Thoughts: Risk of increased suicidal thinking and behavior is higher in adolescents and young adults, especially during the first few weeks of treatment or after a dose change.
- Discontinuation Syndrome: Stopping the tablet suddenly can cause dizziness, irritability, sensory disturbances and flu-like symptoms, so the dose is usually tapered gradually.
- Serotonin Syndrome: Combining escitalopram with other serotonergic drugs can raise serotonin to dangerous levels, causing agitation, rapid heartbeat, high fever and muscle rigidity.
- Bipolar Disorder: Use in undiagnosed bipolar disorder can trigger a manic episode, so a full psychiatric history is important before starting therapy.
