What Experts Say
Irbesartan works by blocking a hormone called angiotensin II, which normally causes blood vessels to narrow. When this hormone is blocked, blood vessels relax and widen, so blood flows more easily and pressure on the artery walls drops. This action helps protect the heart from working too hard over time.
Hydrochlorothiazide works through a different pathway. It signals the kidneys to remove extra salt and water from the body through urine, which lowers the total volume of blood the heart must pump with each beat. Combining these two actions gives a stronger blood pressure response than either medicine used alone.
Doctors often prescribe this combination when blood pressure remains high despite treatment with irbesartan by itself. Regular blood tests are recommended to monitor kidney function and potassium levels, since both medicines affect how the kidneys handle salt, water, and minerals over the long term.
