What Experts Say
Enalapril belongs to the ACE inhibitor class. It blocks the enzyme that converts angiotensin I into angiotensin II, a hormone that narrows blood vessels. This action relaxes the arteries, lowers vascular resistance, and reduces the pressure the heart has to work against.
Hydrochlorothiazide is a thiazide diuretic that acts on the distal convoluted tubule of the kidney. It blocks sodium and chloride reabsorption, which increases urine output and lowers circulating blood volume. This effect works through a different pathway than enalapril, giving added blood pressure control.
Clinical guidelines recommend combination therapy when a single antihypertensive fails to reach target blood pressure. Enalapril and hydrochlorothiazide together lower blood pressure more effectively than either drug alone, while the diuretic also offsets fluid retention that ACE inhibitors can sometimes cause. Regular monitoring of kidney function, potassium, and sodium is still advised during long term use.

- Potassium Supplements: Combined use can raise potassium to dangerous levels.
- Lithium: Co-Renitec can raise lithium levels in the blood and increase toxicity risk.
- Aliskiren: Should not be combined in diabetic or kidney-impaired patients due to added risk of low blood pressure and kidney injury.
- Sacubitril: Must not be started within 36 hours of stopping enalapril due to angioedema risk.
- Diabetes Medicines: Insulin and oral hypoglycemics may need dose adjustment as blood sugar control can change.
- Other Antihypertensives: Additional blood pressure lowering drugs can cause excessive hypotension.
Risks and warnings
- Pregnancy: Can cause fetal injury or death, especially in the second and third trimester, so it should be stopped as soon as pregnancy is confirmed.
- Kidney Function: May reduce kidney function, particularly in patients with pre-existing kidney disease or those on diuretics.
- Angioedema: Rare but serious swelling of the face, lips, tongue, or throat can occur and needs emergency care.
- Low Blood Pressure: First dose hypotension can occur, especially in patients who are dehydrated or on other blood pressure medicines.
- Electrolyte Imbalance: Hydrochlorothiazide can lower sodium and potassium levels, causing weakness or irregular heartbeat.
- Sulfa Allergy: Patients allergic to sulfonamide drugs may react to the hydrochlorothiazide component.
- Diabetes: Should be used cautiously with aliskiren in diabetic patients due to increased risk of kidney problems and high potassium.
