What Experts Say
Secukinumab is a monoclonal antibody that binds directly to interleukin-17A, a signaling protein that drives much of the inflammation seen in psoriasis, psoriatic arthritis and ankylosing spondylitis. By neutralizing IL-17A before it reaches its receptor, the drug interrupts the inflammatory process at an early step.
This targeted approach explains why secukinumab often works faster and more completely on skin plaques than older systemic therapies, and why it also eases joint stiffness and spinal inflammation in the same patient. Because the effect is specific to one cytokine pathway, overall immune suppression is narrower than with broader immunosuppressants, though infection risk still rises.
This selective but still immune modulating action is why tuberculosis screening is required before starting and why live vaccines are avoided during treatment. Loading doses in the first month allow the drug to reach steady blood levels faster, after which monthly dosing maintains that level for continued symptom control.

Drug Interactions
- Live Vaccines: Should not be given during treatment because immune suppression can reduce vaccine safety and effectiveness.
- CYP450 Substrate Drugs (Warfarin, Cyclosporine): Inflammation reduction from secukinumab can normalize CYP450 activity, sometimes requiring a dose adjustment of these medicines.
- Other Biologic or Immunosuppressive Therapies: Combining with other strong immune suppressing treatments raises infection risk and is generally avoided.
Risks and warnings
- Infection Risk: Secukinumab lowers immune defense, increasing the chance of serious bacterial, fungal or viral infections.
- Tuberculosis Screening: A TB test is required before starting treatment, active TB must be treated first.
- Inflammatory Bowel Disease: New or worsening Crohn's disease and ulcerative colitis have been reported, report new stomach pain or diarrhea.
- Vaccination Caution: Live vaccines should be avoided during treatment, discuss vaccination timing with your doctor.
- Allergic Reactions: Rare but serious allergic reactions can occur, seek emergency care for swelling or difficulty breathing.
