What Experts Say
Levofloxacin works by blocking two bacterial enzymes, DNA gyrase and topoisomerase IV, which bacteria need to copy their genetic material. This action is bactericidal, meaning it kills bacteria directly rather than slowing their growth, which is why clinicians often reach for it when infections need fast control.
Physicians generally reserve fluoroquinolones like levofloxacin for infections where first line antibiotics have failed or are unsuitable, since overuse drives resistance and narrows future treatment options. Rational prescribing means matching the drug to a confirmed or strongly suspected bacterial cause and completing the full course rather than stopping once symptoms ease.
The oral tablet has close to complete bioavailability, so patients who start treatment on an intravenous levofloxacin infusion in hospital can often switch to the 250mg tablet once they stabilise and can swallow reliably. This step down approach shortens hospital stays without weakening the therapeutic effect, provided kidney function is monitored appropriately.
