What Experts Say
Cardiologists reserve the 20mg dose of Crescor for patients whose LDL cholesterol remains above target despite lower statin doses, or who carry a very high cardiovascular risk from the outset. This includes people with existing heart disease, diabetes with added risk factors, or a strong family history of early heart attacks.
Rosuvastatin reaches higher blood concentrations in Asian patients compared with Western populations, a pattern confirmed in pharmacokinetic studies. Because of this, doctors typically start therapy at a lower dose and step up to 20mg gradually, watching for muscle and liver related side effects rather than starting every patient directly on the higher strength.
Pakistan carries a heavy burden of premature heart disease, driven by high rates of diabetes, obesity, and inherited lipid disorders. This is why physicians often escalate statin therapy to the 20mg strength once a patient tolerates lower doses well, rather than leaving a high risk patient undertreated.

Drug Interactions
- Cyclosporine: Significantly raises rosuvastatin blood levels, increasing the risk of muscle and kidney side effects, so this combination is generally avoided.
- Gemfibrozil: Increases rosuvastatin levels and the risk of myopathy, so concurrent use is generally avoided or requires a lower rosuvastatin dose.
- Warfarin: Can increase INR and bleeding risk, so clotting levels need closer monitoring when starting or stopping Crescor.
- Antacids: Reduce rosuvastatin absorption, so doses should be separated by at least 2 hours.
- Protease Inhibitors: Certain HIV medicines like lopinavir and ritonavir can raise rosuvastatin levels significantly.
Risks and warnings
- Muscle Pain: Unexplained muscle pain, tenderness, or weakness should be reported right away since it can signal myopathy or, rarely, rhabdomyolysis.
- Liver Function: Doctors usually check liver enzymes before starting the 20mg dose and if symptoms of liver trouble appear.
- Asian Patient Sensitivity: Rosuvastatin blood levels run higher in Asian patients, including many in Pakistan, so doctors monitor response closely at this higher dose.
- Kidney Function: Patients with reduced kidney function may need a lower starting dose than 20mg.
- Pregnancy: Not recommended during pregnancy or while breastfeeding.
