What Experts Say
Ciprofloxacin blocks two bacterial enzymes, DNA gyrase and topoisomerase IV, which bacteria need to copy and repair genetic material. Without them, bacterial cells cannot divide and eventually die. This makes ciprofloxacin bactericidal. It is especially effective against gram negative organisms such as E coli, Klebsiella and Pseudomonas, useful for complicated urinary infections.
Clinicians generally reserve ciprofloxacin for infections needing gram negative coverage or where cultures confirm susceptibility. Overuse of fluoroquinolones has driven rising resistance, so guidelines now favor it as a second line choice for simple cystitis. The full course should be completed even if symptoms improve early, since stopping too soon allows resistance to develop.
Fluoroquinolones carry warnings for tendon rupture, peripheral neuropathy and central nervous system effects such as confusion or seizures. These risks are higher in older adults and those on corticosteroids. Ciprofloxacin can also prolong the QT interval, so caution is needed in patients with heart rhythm problems. Most experts advise using it only when benefit outweighs risk.
