  • Pain from fractures or injuries
  • Chronic pain not controlled by paracetamol alone

  • Lower opioid dose needed compared to tramadol alone
  • Reduces fever alongside pain
  • Convenient two in one tablet reduces pill burden

What Experts Say

Tramadol Hydrochloride 37.5mg acts on opioid receptors and also blocks serotonin and norepinephrine reuptake, giving it a dual pain relieving action. Paracetamol 325mg works mainly in the central nervous system to lower pain signals and reduce fever. Combining the two allows a lower tramadol dose while still achieving strong analgesia.

Pharmacologists note this fixed dose combination produces pain relief faster than either drug alone, since paracetamol has a quicker onset while tramadol's effect builds over time. The combination is generally reserved for pain that does not respond to paracetamol or NSAIDs by themselves, rather than first line, mild discomfort.

Because tramadol carries dependence and seizure risk, clinicians recommend the lowest effective dose for the shortest duration needed. Liver function should be monitored with prolonged use, since paracetamol overdose is a leading cause of drug induced liver injury worldwide.

Drug Class

Drug Interactions

  • MAO Inhibitors: Combining with MAOIs can trigger serotonin syndrome and is contraindicated within 14 days of MAOI use.
  • SSRIs and SNRIs: Concurrent use raises the risk of serotonin syndrome, marked by agitation, high fever and rapid heartbeat.
  • CNS Depressants: Alcohol, benzodiazepines, sedatives and other opioids increase the risk of severe drowsiness and slowed breathing.
  • Carbamazepine: Reduces tramadol's pain relieving effect and can raise seizure risk when stopped or started.
  • Warfarin: Tramadol may enhance the blood thinning effect, so INR should be monitored closely.
  • Other Paracetamol Products: Taking additional paracetamol containing medicines raises the risk of liver toxicity.

Risks and warnings

  • Seizure Risk: Tramadol can lower the seizure threshold, especially in patients with epilepsy or on other seizure risk medicines.
  • Dependence: Long term use can lead to tolerance and opioid dependence, so treatment duration should stay as short as possible.
  • Liver Damage: Exceeding the recommended paracetamol dose or combining with other paracetamol products can cause serious liver injury.
  • Respiratory Depression: Higher doses or combination with alcohol and sedatives can slow or stop breathing, particularly in elderly or debilitated patients.
  • Pregnancy and Breastfeeding: Use only if the doctor confirms the benefit outweighs the risk, since tramadol crosses the placenta and passes into breast milk.
  • Not for Children: Avoid use in children under 12 years due to reports of serious breathing problems.
