What Experts Say
Mefenamic acid belongs to the fenamate group of NSAIDs and works by blocking cyclooxygenase enzymes, which cuts prostaglandin production in the body. Since prostaglandins drive pain, fever, and uterine contractions, this mechanism explains why doctors frequently recommend it for dysmenorrhea, not just general pain relief.
Clinicians generally cap mefenamic acid courses at seven days because prolonged use raises the risk of gastric irritation, ulceration, and kidney strain. The standard adult regimen starts with a 500mg loading dose, followed by 250mg every six hours as pain persists, taken after food to protect the stomach lining.
Doctors advise caution in older adults, patients with reduced kidney function, and those with a history of heart disease, since NSAIDs can affect blood pressure and fluid balance. Pregnant women in the third trimester and patients on regular blood thinners are usually steered toward safer alternatives instead.
