What Experts Say
Levofloxacin is the levo-isomer of ofloxacin and works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication. This gives it lower resistance rates than older fluoroquinolones and activity against many beta-lactam-resistant strains.
Physicians recommend levofloxacin as a step-down option when first-line agents like amoxicillin or macrolides fail, particularly in community-acquired pneumonia. Pakistan carries a high burden of respiratory and urinary infections, and rising resistance to older antibiotics keeps levofloxacin clinically relevant despite growing fluoroquinolone-resistance concerns.
Dosing rationale depends on renal function since levofloxacin is excreted largely unchanged through the kidneys, so doctors adjust dose or interval in reduced creatinine clearance. Monitoring practice includes watching for tendon pain, QT changes in cardiac patients, and glucose shifts in diabetics on sulfonylureas, since these are recognised class effects.
