What Experts Say
Metronidazole works inside anaerobic organisms, where it is chemically activated and then damages the parasite's DNA, killing actively growing amoeba in the gut wall and liver. It reaches tissue levels effective against invasive disease but has limited effect on amoeba cysts sitting in the bowel lumen.
Diloxanide furoate acts directly in the intestinal lumen, where it is partly converted to its active form and interferes with the amoeba's ability to survive and form new cysts. Pairing it with metronidazole is a standard strategy, since it clears the luminal cyst stage that systemic metronidazole does not reliably reach.
This dual mechanism explains why combination therapy lowers relapse rates compared with metronidazole alone. Because absorption and metabolism differ between the two ingredients, treatment courses typically run for several days to address both invasive and luminal stages of infection.
