What Experts Say
Diclofenac sodium works by blocking both COX 1 and COX 2 enzymes, cutting down the prostaglandins that drive pain, swelling, and fever. The injectable form reaches peak blood levels faster than a tablet, which is why doctors reach for it when pain is severe or a patient cannot take anything by mouth.
Clinicians generally limit diclofenac injections to one or two days before switching to an oral NSAID or stopping treatment altogether. Longer injectable courses raise the risk of stomach irritation, kidney strain, and cardiovascular events, so this formulation is meant for short, supervised use rather than daily pain control.
Correct intramuscular technique matters as much as the dose itself. The injection should go deep into a large muscle, ideally the upper outer buttock, and the same site should not be used repeatedly because it can cause local tissue damage. Patients with ulcer history, kidney disease, asthma linked to NSAID sensitivity, or late pregnancy generally need an alternative.

Drug Interactions
- Anticoagulants (Warfarin): Combined use increases the risk of serious bleeding.
- Other NSAIDs or Aspirin: Taking more than one NSAID at a time raises the risk of stomach bleeding without added pain relief.
- ACE Inhibitors or ARBs: Diclofenac can reduce their blood pressure lowering effect and stress the kidneys further.
- Lithium: NSAIDs can raise lithium levels in the blood to a potentially toxic range.
- Methotrexate: Diclofenac can reduce methotrexate clearance, increasing its toxicity.
Risks and warnings
- Gastrointestinal Bleeding: Diclofenac can cause stomach ulcers or bleeding, especially with long use or in people with a history of ulcers.
- Cardiovascular Risk: NSAIDs including diclofenac can raise the risk of heart attack and stroke, particularly with higher doses or existing heart disease.
- Kidney Function: Use with caution in patients with kidney disease or dehydration since NSAIDs can reduce blood flow to the kidneys.
- Injection Site Reactions: Repeated use of the same injection site can cause pain, swelling, or muscle tissue damage.
- Pregnancy: Avoid in the third trimester due to risk of premature closure of the fetal ductus arteriosus.
