About Gouric Tablet 40mg

Gouric is a xanthine oxidase inhibitor tablet containing Febuxostat, used to lower uric acid and prevent gout attacks in adults.

Gouric Tablet Uses

  • Chronic hyperuricemia (persistently elevated blood uric acid levels) in adult patients with gout.
  • Prevention of recurrent acute gout attacks in patients who have experienced two or more attacks per year.
  • Long-term urate-lowering therapy in patients who cannot tolerate Allopurinol or in whom Allopurinol has failed to achieve target uric acid levels.
  • Prevention of uric acid nephrolithiasis (kidney stones caused by uric acid crystal deposition).
  • Tophaceous gout: reduction and eventual dissolution of tophi (uric acid crystal deposits in soft tissues and joints) with sustained urate-lowering therapy.

Gouric Tablet Benefits

  • More potent uric acid lowering than Allopurinol at standard doses, with Febuxostat 80mg achieving target serum urate levels below 6 mg/dL in a significantly higher proportion of patients.
  • Does not require dose adjustment for mild to moderate renal impairment, unlike Allopurinol which requires careful dose reduction based on creatinine clearance.
  • Once-daily dosing regardless of food intake improves adherence in patients requiring lifelong therapy.
  • Effective in patients with Allopurinol intolerance including hypersensitivity syndrome, a rare but potentially life-threatening reaction to Allopurinol.
  • Reduces the frequency of acute gout attacks over time by maintaining consistently low serum urate levels, allowing uric acid crystals in joints and tissues to dissolve gradually.

What Experts Say

Gouric contains Febuxostat, a non-purine selective xanthine oxidase inhibitor and the first new drug class approved for chronic gout management in over 40 years.

How It Works

Febuxostat inhibits xanthine oxidase by binding to its molybdenum pterin active site, blocking both the oxidized and reduced forms of the enzyme. Xanthine oxidase is the terminal enzyme in purine catabolism, responsible for converting hypoxanthine to xanthine and xanthine to uric acid. By blocking this enzyme, Febuxostat reduces the production of uric acid at its source. Unlike Allopurinol, which is a purine analogue that competes with the enzyme substrate, Febuxostat is a non-purine inhibitor that occupies the active site through a different binding interaction. This structural distinction gives Febuxostat higher selectivity for xanthine oxidase and avoids many of Allopurinol's off-target effects on other enzymes in the purine pathway.

The Gout Flare Paradox

A counterintuitive but clinically important fact about urate-lowering therapy is that starting Febuxostat often triggers acute gout attacks in the first 3 to 6 months. This occurs because rapid lowering of serum uric acid causes mobilization and partial dissolution of existing uric acid crystals in joints, releasing crystal fragments that trigger acute inflammatory responses. This is not a sign that the treatment is failing. To prevent these mobilization flares, doctors routinely prescribe prophylactic low-dose Colchicine or an NSAID alongside Gouric for the first 3 to 6 months of treatment.

Clinical Note

In 2019, the FDA required a black box warning on Febuxostat based on the CARES clinical trial, which showed a higher rate of cardiovascular death in patients receiving Febuxostat compared to Allopurinol. Gouric is therefore contraindicated in patients with established ischemic heart disease or congestive heart failure unless Allopurinol is not an option and the treating physician has explicitly assessed and accepted the cardiovascular risk. All patients on Gouric should be monitored for signs of cardiovascular events throughout treatment.

Drug Class

  • Xanthine Oxidase Inhibitor.
  • Urate-Lowering Agent.

Packaging and Quantity

  • 1 pack contains 20 tablets.
  • Available in 40mg and 80mg strengths.

When Not to Use Gouric

  • Known allergy or hypersensitivity to Febuxostat or any ingredient in the tablet.
  • Established ischemic heart disease or congestive heart failure, due to increased cardiovascular death risk identified in the CARES trial.
  • Concurrent use of Azathioprine, Mercaptopurine, or Theophylline, as Febuxostat inhibits their metabolism and can cause life-threatening toxicity.
  • Children and adolescents under 18 years of age.
  • Acute gout attack: Gouric must not be started during an active attack as it can worsen or prolong it.

How to use Gouric Tablet 40mg?

  • Gouric may be taken with or without food.
  • The tablet should be swallowed whole with a full glass of water at the same time each day.
  • Gouric must not be started during an acute gout attack. The attack should be fully resolved before initiating treatment.
  • A doctor will typically prescribe Colchicine or a low-dose NSAID alongside Gouric for the first 3 to 6 months to prevent mobilization gout flares.
  • If a gout attack occurs during treatment, Gouric should not be stopped as this can worsen the flare. The acute attack should be treated separately and Gouric continued.
  • The standard adult starting dose is 40mg once daily. The dose may be increased to 80mg once daily if the serum uric acid target is not achieved after 2 to 4 weeks at 40mg.
  • The dose should not be changed without consulting a doctor. Serum uric acid levels should be tested regularly to confirm the target is being maintained.

Drug Interactions

All current medicines, supplements, and herbal products must be disclosed to the prescribing doctor before starting Gouric. Key interactions include:

  • Azathioprine and Mercaptopurine: This is a contraindicated combination. Xanthine oxidase metabolizes both drugs. Inhibiting this enzyme with Febuxostat causes dangerous accumulation of these medicines, leading to severe bone marrow suppression and potentially fatal toxicity.
  • Theophylline: Xanthine oxidase is involved in Theophylline metabolism. Febuxostat significantly raises Theophylline blood levels, increasing the risk of Theophylline toxicity including seizures and cardiac arrhythmias.
  • Colchicine: No pharmacokinetic interaction, but they are commonly prescribed together for gout flare prophylaxis. No dose adjustment required.
  • NSAIDs and Aspirin: No direct pharmacokinetic interaction. Co-prescribed for flare prophylaxis or analgesia during mobilization attacks.
  • Warfarin: There is limited evidence of interaction but INR should be monitored when starting or stopping Febuxostat in patients on warfarin.
  • Didanosine: Febuxostat inhibits xanthine oxidase which is involved in Didanosine metabolism. Concurrent use may raise Didanosine levels. Monitoring is required.

Risks and warnings

  • Cardiovascular death risk (FDA Black Box Warning): The CARES trial showed a significantly higher rate of cardiovascular death in Febuxostat-treated patients compared to Allopurinol. Gouric is contraindicated in patients with ischemic heart disease or congestive heart failure unless Allopurinol is not an option.
  • Contraindicated drug combinations: Concurrent use of Azathioprine, Mercaptopurine, or Theophylline with Febuxostat is contraindicated. The inhibition of xanthine oxidase causes dangerous accumulation of these medicines that can be fatal. All current medicines must be disclosed before starting Gouric.
  • Gout flare on initiation: Starting Gouric commonly triggers acute gout attacks in the first 3 to 6 months. This is a predictable pharmacological response, not a sign of treatment failure. Prophylactic Colchicine or NSAID therapy for this period is strongly recommended.
  • Liver toxicity: Febuxostat has been associated with elevated liver enzymes and rare cases of serious liver injury. Liver function should be measured before starting treatment and monitored during therapy.
  • Do not start during an acute attack: Gouric must not be initiated during an active gout flare. Starting or stopping urate-lowering therapy during an attack worsens and prolongs it.
  • Kidney disease: Gouric can be used in mild to moderate renal impairment without dose adjustment. It should be used with caution in severe renal impairment (creatinine clearance below 30 mL per minute), as clinical data in this group are limited.
  • Pregnancy: The safety of Febuxostat during pregnancy has not been established. It should not be used during pregnancy.
  • Breastfeeding: Febuxostat may pass into breast milk. Breastfeeding is not recommended during treatment.
  • Driving and machinery: Gouric may cause dizziness in some patients. Caution is advised when driving or operating heavy machinery.

Gouric Tablet Side Effects

Most patients tolerate Gouric well at recommended doses. The most clinically important side effect to monitor is the mobilization gout flare in the first 3 to 6 months of treatment, which is expected and not a reason to stop the medicine.

  • Acute gout flares in the first 3 to 6 months of therapy due to uric acid crystal mobilization.
  • Nausea or mild stomach discomfort.
  • Liver enzyme elevation on blood tests, usually mild and asymptomatic.
  • Headache or dizziness.
  • Skin rash or itching.
  • Joint pain (arthralgia) unrelated to gout.

Serious side effects are rare. Medical attention should be sought immediately if any of the following occur:

  • Chest pain, shortness of breath, or symptoms of a heart attack or stroke, in light of the FDA cardiovascular safety warning.
  • Yellowing of the skin or eyes, severe abdominal pain, or dark urine, which may indicate serious liver damage.
  • Signs of a severe allergic reaction including facial swelling, difficulty breathing, or widespread rash.
  • Unusual bruising or signs of bone marrow suppression, which may indicate an undetected interaction with Azathioprine or Mercaptopurine.

Storage Conditions

  • Store the medicine at room temperature
  • Keep out of reach and sight of children
  • Protect it from sunlight and moisture
