What Experts Say
Levofloxacin belongs to the fluoroquinolone class of antibiotics. It blocks two bacterial enzymes, DNA gyrase and topoisomerase IV, which bacteria need to copy their genetic material. Without these enzymes working, bacteria cannot multiply and eventually die off. This gives levofloxacin activity against a broad range of gram positive and gram negative organisms.
Doctors often prescribe levofloxacin when first line antibiotics have failed or when the infection responds well to fluoroquinolones. Its ability to reach high concentrations in lung tissue, urinary tract and skin makes it a preferred option for respiratory, urinary and skin infections. Once daily dosing also improves patient compliance.
Fluoroquinolones including levofloxacin carry safety concerns such as tendon damage, nerve problems and heart rhythm changes. Medical guidelines recommend reserving levofloxacin for infections where safer antibiotics are not suitable. Overuse also contributes to antibiotic resistance, so treatment duration and dose should always follow a doctor's assessment rather than self medication.

- QT Prolonging Drugs: Combining with antiarrhythmics or antipsychotics that prolong the QT interval raises the risk of serious heart rhythm problems.
Risks and warnings
- Tendon Damage: Levofloxacin can cause tendinitis and tendon rupture, especially in the Achilles tendon, more often in people over 60 or those taking steroids.
- Nerve Damage: Peripheral neuropathy may develop and can become permanent, so treatment should stop if tingling, numbness or weakness occurs.
- Nervous System Effects: Seizures, confusion, anxiety and rarely hallucinations have been reported, particularly in people with a history of seizure disorders.
- Heart Rhythm Risk: This medicine can prolong the QT interval and should be used cautiously in people with existing heart rhythm problems.
- Myasthenia Gravis: Levofloxacin can worsen muscle weakness in people with myasthenia gravis and should generally be avoided in this condition.
