Cephradine belongs to the first generation cephalosporin group of antibiotics. It works by binding to penicillin binding proteins in the bacterial cell wall, which blocks the final step of cell wall synthesis. This weakens the wall until the bacterial cell ruptures and dies.

The drug covers many gram positive organisms such as streptococci and methicillin sensitive staphylococci, along with some gram negative bacteria including E. coli and Klebsiella.
