What Experts Say
Levofloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes bacteria need to replicate their genetic material. This broad mechanism explains its activity against a wide range of gram positive and gram negative organisms, including several that resist older antibiotic classes.
Because of tendon, nerve, and heart rhythm safety concerns, most guidelines now position fluoroquinolones like levofloxacin as second-line therapy, reserved for infections where narrower-spectrum antibiotics have failed or are contraindicated, rather than a first choice for routine infections.
Rising antibiotic resistance in Pakistan makes culture-guided prescribing increasingly important, so doctors generally try to confirm the specific bacteria and its sensitivity pattern before relying on a broad-spectrum agent like this one. Overuse of fluoroquinolones for minor infections is a recognized driver of resistance that clinicians are being asked to avoid.
