What Experts Say
Tizanidine works differently from relaxants like baclofen. It stimulates alpha 2 adrenergic receptors in the spinal cord, reducing release of chemical messengers that keep motor neurons overactive. This lowers muscle tone and eases spasm without directly weakening the muscle, so patients often notice looser movement rather than reduced strength.
The short half life of tizanidine, generally two to four hours, means blood pressure lowering and sedative effects appear soon after a dose and fade fairly quickly. Doctors often time doses around physiotherapy sessions or bedtime rather than spacing them evenly through the day. A low starting dose reduces the risk of a sudden blood pressure drop.
Tizanidine is broken down mainly by a liver enzyme called CYP1A2, so combining it with drugs that block this enzyme can raise blood levels several times over and cause dangerous hypotension or extreme drowsiness. This is why doctors avoid pairing it with certain antibiotics or antidepressants. Liver function is usually checked periodically during longer treatment.
