What Experts Say
Tizanidine acts as a central alpha-2 adrenergic agonist. It reduces the release of excitatory neurotransmitters from spinal interneurons. This action decreases muscle tone and eases spasticity without causing significant muscle weakness in most patients.
Clinical evidence supports its use for spasticity linked to multiple sclerosis and spinal cord injury, as well as for painful muscle spasms of musculoskeletal origin. The short acting nature of Tizanidine allows dosing to be timed around periods of greatest functional need, though the sustained release formulation extends coverage across the day.
Because Tizanidine undergoes extensive liver metabolism through the CYP1A2 pathway, drug interactions and hepatic monitoring matter clinically. Sudden withdrawal after prolonged use can trigger rebound hypertension and a fast heart rate, so dose tapering is recommended. Blood pressure and liver enzymes should be checked periodically during treatment.
