What Experts Say
Domperidone works outside the brain's protective barrier, at a control centre called the chemoreceptor trigger zone. This dopamine blockade calms the vomiting reflex without the drowsiness or movement side effects that older antiemetics cause. This selective action is why doctors favor it for functional dyspepsia and diabetic gastroparesis.
Physicians keep the dose at the lowest effective level and for the shortest time, usually no more than seven days without review. This caution exists because domperidone can lengthen the heart's QT interval and raise the risk of an irregular heartbeat, a risk that climbs past age sixty or once the daily dose passes 30 mg.
Domperidone is broken down by the same liver enzyme pathway as several common antibiotics and antifungals, so prescribers check a patient's full medicine list first, since combining it with drugs like ketoconazole or erythromycin can raise blood levels and cardiac risk. Gastroparesis and functional dyspepsia are common complaints in general medicine and gastroenterology clinics across Pakistan.

Drug Interactions
- Azole Antifungals: Ketoconazole, fluconazole and voriconazole raise domperidone levels and increase the risk of an irregular heartbeat.
- Macrolide Antibiotics: Erythromycin, clarithromycin and telithromycin can raise domperidone blood levels and add to cardiac risk.
- Antiarrhythmic Drugs: Amiodarone, quinidine, disopyramide, dofetilide and sotalol combined with domperidone can further prolong the QT interval.
- Calcium Channel Blockers: Diltiazem and verapamil can add to the QT prolonging effect of domperidone.
- Antipsychotics: Haloperidol, pimozide and sertindole together with domperidone increase the chance of heart rhythm problems.
- SSRIs: Citalopram and escitalopram carry their own QT prolonging effect that can add to domperidone's cardiac risk.
- Other Prokinetics: Cisapride, dolasetron and prucalopride should be avoided together with domperidone due to overlapping heart rhythm risk.
- HIV Protease Inhibitors: These medicines raise domperidone levels in the blood and should be avoided together.
Risks and warnings
- Heart Rhythm Risk: Domperidone can prolong the heart's QT interval and raise the risk of a serious irregular heartbeat, a risk that is higher in people over 60 years or on doses above 30mg a day.
- Dose Limit: Do not exceed three tablets (30mg) in 24 hours, since higher doses raise cardiac risk without adding meaningful benefit.
- Duration Limit: Treatment is usually kept to seven days or less, longer use should only continue under a doctor's supervision.
- Heart Conditions: Avoid this medicine with a known heart rhythm disorder, heart failure, or an ECG showing a prolonged QT interval.
- Electrolyte Imbalance: Low potassium or magnesium, or high potassium levels, can increase the chance of an abnormal heartbeat during treatment.
- Liver Disease: Avoid use in moderate to severe liver impairment, since the body clears domperidone more slowly and drug levels can build up.
- Bowel Blockage: Do not use with a bowel obstruction, perforation, or bleeding in the stomach or intestines.
- Pregnancy And Breastfeeding: Use only if a doctor considers it clearly necessary, since small amounts can pass into breastmilk.
