About NIB Tablet

NIB Tablet is a nebivolol based beta blocker used mainly to control high blood pressure.

NIB Tablet Uses

  • Long term control of high blood pressure in adults
  • Add on treatment of stable chronic heart failure in older patients
  • Slowing an abnormally fast resting heart rate driven by excess adrenaline activity
  • Reducing the workload on a heart that is already under strain
  • Blood pressure control in patients who cannot tolerate the dry cough caused by ACE inhibitors
  • Supportive control of chest pain linked to raised blood pressure, when a doctor advises it

NIB Tablet Benefits

  • Once daily dosing that is easy to remember and stick with
  • Highly selective action on the heart, so breathing side effects are less common than with older beta blockers
  • Extra vessel relaxation through nitric oxide release, which helps blood flow more freely
  • Smooth blood pressure cover across a full 24 hours from a single tablet
  • Largely neutral effect on blood sugar and cholesterol readings in most patients
  • Lower long term risk of stroke, kidney damage and heart attack when blood pressure stays controlled

What Experts Say

Nebivolol is called a third generation beta blocker because it does two jobs at once. It blocks beta 1 receptors in the heart, which slows the pulse and reduces the force of each beat, and it also prompts the lining of blood vessels to release nitric oxide. That second action widens the vessels, so pressure falls without the cold hands and heavy fatigue older beta blockers often cause.

Its selectivity for beta 1 receptors is the highest in the class at usual doses, which is why physicians often prefer it for patients with mild airway disease or metabolic concerns. Selectivity fades as the dose rises, so doctors keep the dose at the lowest level that holds the target. In chronic heart failure it is started very low and raised slowly over weeks, with pulse and pressure checked at each step.

Pakistan carries a heavy burden of uncontrolled hypertension, and many patients stop treatment once they feel well. Doctors stress that this medicine controls blood pressure rather than curing it, and that stopping suddenly can trigger a sharp rebound.

Drug Class

  • Beta adrenergic blocking agent (cardioselective)
  • Antihypertensive
  • Vasodilating third generation beta blocker

How to use NIB Tablet?

  • Take the tablet exactly as your doctor prescribes and never adjust the dose on your own
  • Swallow it whole with a glass of water, with or without food
  • Take it at the same time each day, ideally in the morning, to keep a steady level in the blood
  • If you miss a dose, take it when you remember, but skip it if your next dose is near
  • Never take two tablets together to make up for a missed one
  • Do not stop the medicine suddenly, even if you feel completely fine, because your doctor must taper it over one to two weeks
  • Keep a record of home blood pressure and pulse readings to show at follow up visits

Drug Interactions

  • Verapamil and diltiazem: Combining them can slow the heart dangerously and depress its pumping strength.
  • Other antihypertensives and nitrates: Blood pressure may drop too far, causing dizziness or fainting.
  • Antiarrhythmic drugs such as amiodarone or digoxin: The risk of heart block and very slow pulse increases.
  • Insulin and oral antidiabetics: Warning signs of low blood sugar can be hidden, so glucose needs closer watching.
  • Fluoxetine, paroxetine and similar antidepressants: They slow the breakdown of nebivolol and raise its blood levels.
  • NSAIDs such as ibuprofen and diclofenac: Regular use can blunt the blood pressure lowering effect.

Risks and warnings

  • Sudden withdrawal: Stopping the tablet abruptly can cause a rebound rise in blood pressure, chest pain or an irregular heartbeat.
  • Slow heart rate: Tell your doctor if your resting pulse drops below 50 beats per minute or you feel faint.
  • Asthma and COPD: Patients with active wheezing need caution, since higher doses lose their heart selectivity and may tighten the airways.
  • Diabetes: The medicine can mask the shakiness and fast pulse that normally warn of low blood sugar, so check glucose more often.
  • Liver disease: Severe liver impairment slows clearance of nebivolol, so the drug is generally avoided in these patients.
  • Heart block and severe heart failure: It must not be used in untreated second or third degree heart block, or in decompensated failure needing intravenous support.
  • Surgery and anaesthesia: Inform your surgeon and anaesthetist beforehand, because the drug alters the heart response during procedures.
  • Pregnancy and breastfeeding: Use only if a doctor judges it necessary, as safety data in pregnancy and nursing remain limited.
