What Experts Say
Piroxicam gel belongs to the topical NSAID group, which international arthritis guidelines recommend as a first choice for osteoarthritis of superficial joints such as the knees and hands. Clinical studies show that piroxicam applied to the skin reaches the joint tissues underneath while blood levels stay near 5 percent of those seen with oral capsules.
According to rheumatology reviews, this low systemic exposure means far fewer stomach, kidney, and heart problems than oral NSAIDs cause. Local skin reactions and photosensitivity remain the main concerns, so specialists advise applying the gel only to intact skin, keeping treated areas out of strong sunlight, and reviewing treatment after about 4 weeks of use.
In Pakistan, physicians often suggest topical piroxicam for older adults with painful knees and for workers with repeated muscle strain, because it limits total drug exposure in people who already take several medicines. They stress that pain lasting beyond a week, or a joint that is swollen and warm, needs proper medical assessment instead of continued self treatment.

Drug Interactions
- Oral NSAIDs: Combining the gel with ibuprofen, diclofenac, or aspirin tablets raises the overall NSAID load and side effect risk.
- Anticoagulants: Blood thinners such as warfarin need caution, since absorbed piroxicam can add slightly to bleeding risk.
- Diuretics and blood pressure medicines: Heavy application over large areas may blunt their effect through NSAID activity.
- Other topical products: Creams, sunscreens, or cosmetics layered on the treated area can change gel absorption, so leave a gap between them.
- Methotrexate and lithium: Extensive long-term use of the gel could reduce clearance of these medicines, so tell your doctor if you take them.
Risks and warnings
- Allergy alert: Do not use this gel if aspirin or other NSAIDs have ever triggered asthma, hives, or a runny nose in you.
- Broken skin: Never apply the gel to cuts, wounds, infected skin, or areas affected by eczema.
- Sun sensitivity: Treated skin can react to sunlight, so keep the area covered or away from strong sun and tanning lamps.
- Pregnancy: Avoid this gel in the third trimester, and ask a doctor before use at any stage of pregnancy or while breastfeeding.
- Extensive use: Applying large amounts over big areas for long periods can absorb enough drug to cause NSAID side effects.
- Existing conditions: Check with your doctor first if you have asthma, stomach ulcers, or kidney disease.
- Children: This gel is not recommended for anyone under 18 unless a doctor advises it.
