What Experts Say
Domperidone blocks dopamine receptors in the gut wall and in the brain's chemoreceptor trigger zone, the area that senses toxins and triggers vomiting. This dual action calms the vomiting reflex while also tightening the lower esophageal sphincter and speeding up stomach emptying after meals.
Because domperidone does not cross the blood brain barrier easily, it causes far less sedation and fewer movement related side effects than older antiemetics like metoclopramide. This makes it a preferred choice for functional dyspepsia and diabetic gastroparesis in many treatment guidelines worldwide.
Its main safety concern is a dose related effect on heart rhythm through mild QT interval prolongation, which can rarely trigger a dangerous arrhythmia. Prescribers usually limit the dose and duration, and avoid combining it with other drugs that carry the same cardiac risk, especially in older adults.

Drug Interactions
- Ketoconazole and azole antifungals: Raise domperidone blood levels and increase the risk of heart rhythm problems.
- Macrolide antibiotics like erythromycin: Block the enzyme that clears domperidone, increasing cardiac risk.
- Other QT prolonging drugs: Combining with antiarrhythmics or certain antipsychotics adds to the heart rhythm risk.
- Anticholinergic drugs: Can reduce the stomach emptying effect of domperidone.
Risks and warnings
- Heart rhythm: Domperidone can prolong the QT interval, so it is avoided in people with existing heart rhythm problems or on other QT prolonging drugs.
- Liver disease: Needs dose adjustment since domperidone is broken down by the liver.
- Kidney disease: Requires a lower dose or longer dosing interval in moderate to severe kidney impairment.
- Pregnancy: Use only if a doctor decides the benefit outweighs the risk, since safety data in pregnancy is limited.
- Breastfeeding: Small amounts pass into breast milk, so use under medical supervision.
- Elderly patients: Carry a higher risk of heart rhythm side effects and need the lowest effective dose.
