What Experts Say
Cilostazol works by inhibiting phosphodiesterase III, which raises cyclic AMP levels inside platelets and vascular smooth muscle cells. This dual action reduces platelet aggregation while relaxing blood vessels, together improving blood flow to the legs during exertion.
Physicians typically prescribe Cilostazol at 100mg twice daily, reserving the 50mg strength for patients who need a lower starting dose or who are taking medicines that slow its breakdown. Vascular specialists caution that visible improvement in walking distance can take several weeks of consistent use, so early discontinuation is a common reason for perceived treatment failure.
Because Cilostazol carries a documented mortality signal in patients with heart failure, prescribers screen cardiac history carefully before starting therapy and avoid the drug in anyone with reduced heart pump function. Regular follow up also checks for bleeding tendencies, since the antiplatelet effect can add to the risk from aspirin or other blood thinners taken for the same vascular condition.

- Liver Or Kidney Impairment: Dose adjustment or avoidance may be needed since Cilostazol is metabolized by the liver and cleared partly by the kidneys
