How Paracetamol Works

Paracetamol acts centrally and peripherally to reduce pain and fever.

It inhibits the synthesis of prostaglandins in the central nervous system by blocking COX enzymes, raising the pain threshold and reducing the perception of pain at the brain level.

Unlike NSAIDs, Paracetamol does not inhibit peripheral COX enzymes significantly, which means it does not cause gastrointestinal irritation or platelet dysfunction at standard doses.

How Orphenadrine Works

Orphenadrine Citrate is a centrally acting skeletal muscle relaxant with a unique dual mechanism.

It antagonizes NMDA receptors in the spinal cord and brain, blocking the excitatory glutamate signaling that sustains painful muscle spasms. It also has anticholinergic and antihistaminic properties that contribute to its muscle-relaxing and mild analgesic effects.

By acting centrally rather than directly on muscle tissue, Orphenadrine breaks the pain-spasm cycle without causing the neuromuscular blockade seen with peripheral muscle relaxants.

Clinical Note

The Forte formulation is intended for adults with moderate to severe musculoskeletal pain where the standard strength is insufficient.

Orphenadrine has anticholinergic properties that can cause dry mouth, urinary retention, and blurred vision, particularly in elderly patients and those with prostatic hypertrophy or glaucoma.

Provas-N Forte should not be used as a long-term muscle relaxant. It is appropriate for short-term management of acute pain episodes.

How to use Provas-N Forte Tablet?

• Provas-N Forte should be taken with food or milk to reduce the risk of stomach upset.
• The tablet should be swallowed whole with a full glass of water.
• Alcohol must be avoided throughout treatment as it increases the sedative effect of Orphenadrine and raises the risk of Paracetamol-induced liver damage.
• This medicine should be used for the shortest time necessary to control symptoms.
• The standard adult dose is 1 tablet two to three times daily as directed by the doctor, not exceeding 3 tablets in 24 hours.
• The dose should not be increased or the course extended without consulting a doctor.

Drug Interactions

All current medicines, supplements, and herbal products should be disclosed to the prescribing doctor before starting Provas-N Forte. Key interactions include:

• Alcohol: Concurrent use enhances the sedative effect of Orphenadrine and significantly raises the risk of Paracetamol hepatotoxicity. Strictly contraindicated during treatment.
• MAO inhibitors: Concurrent use with Orphenadrine is contraindicated due to the risk of dangerous blood pressure elevation and CNS toxicity.
• Warfarin: Paracetamol at regular doses may enhance the anticoagulant effect of warfarin, increasing bleeding risk. INR should be monitored closely.
• CNS depressants including benzodiazepines, opioids, sedatives, and antihistamines: Orphenadrine adds to CNS depression, increasing the risk of excessive sedation.
• Anticholinergic medicines: Additive anticholinergic effects including urinary retention, dry mouth, blurred vision, and constipation.
• Other Paracetamol-containing medicines: Concurrent use creates a significant risk of Paracetamol overdose and liver damage. Only one Paracetamol-containing product should be taken at a time.
• Phenytoin and other CYP enzyme inducers: May increase the rate of conversion of Paracetamol to its hepatotoxic metabolite, raising liver damage risk even at standard doses.

Risks and warnings

• Paracetamol overdose: Exceeding the recommended dose of Provas-N Forte can cause serious liver damage. Only one Paracetamol-containing product should be used at a time.
• Liver disease: Paracetamol is metabolized by the liver. Patients with liver impairment must use the lowest effective dose under close medical supervision.
• Alcohol: Combining alcohol with Paracetamol significantly raises the risk of hepatotoxicity. Combining alcohol with Orphenadrine increases sedation. Both interactions are clinically serious.
• Glaucoma: Orphenadrine's anticholinergic effect raises intraocular pressure and is contraindicated in patients with narrow-angle glaucoma.
• Prostatic hypertrophy: Orphenadrine can cause or worsen urinary retention in men with an enlarged prostate.
• Elderly patients: Older adults are more sensitive to the anticholinergic effects of Orphenadrine including confusion, dry mouth, urinary retention, and constipation.
• Pregnancy: Provas-N Forte is not recommended during pregnancy. Both Paracetamol and Orphenadrine should only be used under direct medical supervision if clinically necessary.
• Breastfeeding: Both active ingredients pass into breast milk. A doctor should be consulted before taking Provas-N Forte during breastfeeding.
• Driving and machinery: Orphenadrine causes drowsiness and dizziness. Driving and operating heavy machinery must be avoided during treatment.

Provas-N Forte Tablet Side Effects

Most patients tolerate Provas-N Forte well at recommended doses. Side effects are generally mild and related to the anticholinergic properties of Orphenadrine.

• Dry mouth.
• Mild drowsiness or sedation.
• Dizziness or lightheadedness.
• Blurred vision.
• Nausea or mild stomach discomfort.
• Urinary hesitancy or difficulty urinating.

Serious side effects are rare. Medical attention should be sought immediately if any of the following occur:

• Signs of Paracetamol overdose or liver damage including nausea, vomiting, abdominal pain, and yellowing of the skin or eyes.
• Confusion, agitation, or hallucinations, particularly in elderly patients.
• Rapid or irregular heartbeat.
• Complete inability to urinate.
• Severe allergic reaction including facial swelling, difficulty breathing, or widespread rash.
