What Experts Say
Domperidone works as a peripheral dopamine D2 receptor antagonist. It increases the tone and contractions of the stomach and upper intestine, helping food move through the digestive tract more efficiently. This mechanism explains why it works well for nausea, vomiting and the bloated, heavy feeling many patients describe after meals.
Unlike some antiemetics, domperidone does not cross the blood brain barrier easily, so it carries a lower risk of the drowsiness and movement related side effects seen with older antinausea drugs. Its main safety concern relates to the heart, since higher doses can affect the QT interval and increase the chance of an irregular heartbeat.
Because of this cardiac risk, most guidelines now recommend the lowest effective dose for the shortest duration needed, along with caution in people with existing heart conditions or those taking other medicines that affect heart rhythm. Regular review by a doctor is advised for anyone using domperidone beyond a short course.
