What Experts Say
Paracetamol works mainly through the central nervous system. It lowers the brain's temperature set point and reduces pain signals by limiting prostaglandin production there. Unlike NSAIDs, it has little effect on prostaglandins in peripheral tissues, so it does not carry the same risk of stomach irritation or bleeding.
Doctors in Pakistan often prefer paracetamol as a first choice for fever and mild pain, especially during dengue season when NSAIDs and aspirin raise bleeding risk in an illness that already lowers platelet counts. It is also considered safer for pregnant women, older adults with a history of ulcers, and patients taking blood thinners.
The main caution with paracetamol is the narrow gap between an effective dose and a liver damaging one. Many cold, flu, and combination pain products already contain paracetamol, so adding a plain tablet on top can push a patient past the safe daily limit without realizing it. Physicians advise checking every label and never exceeding four grams in twenty four hours.
