What Experts Say
Levofloxacin belongs to the fluoroquinolone class of antibiotics. It works by inhibiting two bacterial enzymes, DNA gyrase and topoisomerase IV, which bacteria need to replicate and repair genetic material. Blocking these enzymes stops bacterial division and causes bacterial death, making the drug bactericidal rather than merely bacteriostatic.
Physicians favor levofloxacin for respiratory and urinary tract infections because it reaches high concentrations in lung tissue and urine relative to blood levels. Its broad spectrum activity covers many gram positive and gram negative organisms, though resistance varies by region. Experts stress that fluoroquinolones should be reserved for infections where safer first line antibiotics are unsuitable.
Regulatory agencies have issued boxed warnings for fluoroquinolones because of tendon rupture, peripheral nerve damage and central nervous system effects that can occur even after a single dose. Specialists recommend caution in older adults and anyone taking corticosteroids, since this combination further raises tendon injury risk.
