About Septran DS Tablet 800mg/160mg

Septran is a broad-spectrum combination antibiotic used to treat a wide range of bacterial infections.

Septran Tablet Uses

  • Urinary tract infections (UTIs) including cystitis, urethritis, and uncomplicated pyelonephritis.
  • Acute and chronic bronchitis caused by susceptible bacteria.
  • Community-acquired pneumonia in mild to moderate cases.
  • Pneumocystis jirovecii pneumonia (PCP) treatment and prophylaxis in immunocompromised patients.
  • Acute otitis media (middle ear infections) caused by Haemophilus influenzae or Streptococcus pneumoniae.
  • Acute sinusitis.
  • Traveler's diarrhea caused by Escherichia coli.
  • Typhoid fever and Salmonella infections.
  • Gastrointestinal infections including Shigellosis (bacillary dysentery).
  • Skin and soft tissue infections caused by susceptible organisms, including methicillin-susceptible Staphylococcus aureus (MSSA).
  • Prophylaxis against Pneumocystis pneumonia in HIV-positive patients and transplant recipients.

Septran Tablet Benefits

  • The DS (Double Strength) formulation provides twice the active ingredient dose of standard Septran in a single tablet, improving convenience for twice-daily dosing in moderate to severe infections.
  • Effective against both gram-positive and gram-negative bacteria, giving it broad clinical utility across respiratory, urinary, and gastrointestinal infections.
  • One of the few oral antibiotics effective for Pneumocystis jirovecii pneumonia, a critical indication in immunocompromised patients.
  • Well established for UTI treatment, particularly in patients where resistance to first-line agents like amoxicillin has been documented locally.
  • Affordable and widely available across Pakistan, making it accessible for long-course treatment such as UTI prophylaxis.

What Experts Say

Septran contains Co-Trimoxazole, a synergistic combination of Sulfamethoxazole and Trimethoprim that targets bacterial folic acid synthesis at two sequential steps, producing a bactericidal effect greater than either agent alone.

How It Works

Bacteria must synthesize their own folic acid because, unlike human cells, they cannot absorb it from the environment. Sulfamethoxazole competitively inhibits dihydropteroate synthase (DHPS), blocking the first step in folic acid synthesis. Trimethoprim inhibits dihydrofolate reductase (DHFR), blocking the second and immediately downstream step. By simultaneously blocking two consecutive enzymes in the same pathway, Co-Trimoxazole produces a synergistic bactericidal effect. Human cells are not affected because they rely on dietary folate and use a form of DHFR that Trimethoprim binds with far lower affinity than the bacterial enzyme.

DS vs Standard Strength

Septran DS contains exactly double the active ingredient amounts of standard Septran. Standard Septran (480mg) contains Sulfamethoxazole 400mg and Trimethoprim 80mg. Septran DS (960mg) contains Sulfamethoxazole 800mg and Trimethoprim 160mg. For most adult infections, one DS tablet twice daily achieves equivalent blood levels to two standard tablets twice daily, reducing the pill burden for patients on extended treatment courses.

Clinical Note

Co-Trimoxazole resistance is rising significantly in Pakistan, particularly for UTI-causing E. coli, limiting its empirical use for uncomplicated UTIs in areas with high local resistance rates. Sensitivity testing is strongly recommended before prescribing Septran for recurrent or complicated infections. Septran must not be used in patients with folate deficiency, as it further impairs folate metabolism. Adequate fluid intake throughout treatment is essential to prevent crystalluria, a known but rare adverse effect at standard doses.

How to use Septran DS Tablet 800mg/160mg?

  • Septran should be taken with food or immediately after a meal to reduce the risk of nausea and stomach upset.
  • The tablet should be swallowed whole with a full glass of water.
  • Fluid intake should be increased during treatment to maintain adequate hydration and reduce the risk of crystalluria.
  • The full prescribed course must be completed even if symptoms improve before it ends. Stopping early risks treatment failure and resistance.
  • The standard adult dose for most infections is 1 DS tablet (960mg) twice daily for 5 to 14 days depending on the infection type and severity as directed by the doctor.
  • The dose should not be changed or extended without consulting a doctor.

Drug Interactions

All current medicines, supplements, and herbal products must be disclosed before starting Septran. Key interactions include:

  • Warfarin: Co-Trimoxazole significantly enhances the anticoagulant effect of warfarin by inhibiting its metabolism and displacing it from plasma proteins. INR must be monitored closely.
  • Methotrexate: Trimethoprim inhibits dihydrofolate reductase, the same enzyme targeted by methotrexate, dramatically increasing its toxicity risk including bone marrow suppression. Concurrent use is contraindicated.
  • Phenytoin: Sulfamethoxazole inhibits phenytoin metabolism, raising its blood levels and the risk of phenytoin toxicity.
  • ACE inhibitors and potassium-sparing diuretics: Trimethoprim reduces kidney potassium excretion. Combining with ACE inhibitors, ARBs, or potassium-sparing diuretics significantly increases the risk of life-threatening hyperkalemia.
  • Oral antidiabetic agents such as glibenclamide and tolbutamide: Co-Trimoxazole may enhance their blood glucose-lowering effect, increasing the risk of hypoglycemia.
  • Ciclosporin: Co-Trimoxazole can reduce ciclosporin blood levels and increase the risk of nephrotoxicity.
  • Dapsone: Concurrent use increases blood levels of both drugs and raises the risk of methemoglobinemia.
  • Oral contraceptives: Co-Trimoxazole may reduce the effectiveness of hormonal contraceptives by altering gut flora. Additional contraceptive precautions are advised.
  • Pyrimethamine: Concurrent use increases the risk of megaloblastic anemia due to combined antifolate effects.

Risks and warnings

  • Severe skin reactions: Co-Trimoxazole is one of the most common drug causes of Stevens-Johnson syndrome and toxic epidermal necrolysis, which can be life-threatening. Any skin rash developing during treatment must be assessed immediately and Septran stopped at once.
  • Antibiotic resistance: Co-Trimoxazole resistance is rising significantly in Pakistan. Empirical use for UTIs should be guided by local resistance data. Sensitivity testing is strongly recommended for recurrent infections.
  • Hyperkalemia: Trimethoprim reduces potassium excretion from the kidney. The risk of dangerously high potassium levels is significantly increased when Septran is combined with ACE inhibitors, ARBs, or potassium-sparing diuretics.
  • Bone marrow suppression: Prolonged use can suppress blood cell production, causing anemia, leukopenia, or thrombocytopenia. Blood counts should be monitored in patients on extended courses.
  • Kidney disease: Septran is primarily excreted through the kidneys. Dose reduction is required in renal impairment. Adequate hydration must be maintained throughout treatment to prevent crystalluria.
  • Liver disease: Both Sulfamethoxazole and Trimethoprim are metabolized by the liver. Patients with significant hepatic impairment should use Septran with caution and under close monitoring.
  • Folate deficiency: Septran further impairs folate metabolism. It must not be used in patients with pre-existing folate deficiency anemia. Folic acid supplementation may be required in patients on long courses.
  • Pregnancy: Co-Trimoxazole is contraindicated in the first trimester due to the risk of neural tube defects related to folate antagonism, and at term due to the risk of neonatal hemolysis and jaundice.
  • G6PD deficiency: Sulfamethoxazole can trigger hemolytic anemia in patients with G6PD deficiency. A G6PD screen is advisable before starting long-term Septran therapy.
  • Photosensitivity: Septran increases sensitivity to sunlight. Prolonged sun exposure should be avoided, and broad-spectrum sunscreen should be used during outdoor activity.
  • Driving and machinery: Septran may cause dizziness in some patients. Caution is advised when driving or operating heavy machinery.

Septran Tablet Side Effects

Most patients tolerate Septran well at recommended doses and with adequate hydration. Side effects are more common with the DS formulation due to higher drug concentrations.

  • Nausea or vomiting.
  • Loss of appetite.
  • Diarrhea or loose stools.
  • Headache or dizziness.
  • Skin rash or itching.
  • Increased sensitivity to sunlight (photosensitivity), leading to sunburn more easily than usual.

Serious side effects are rare but require immediate medical attention:

  • Stevens-Johnson syndrome or toxic epidermal necrolysis presenting as blistering, peeling, or widespread rash affecting skin and mucous membranes. This is a medical emergency.
  • Severe skin reactions including erythema multiforme.
  • Signs of blood disorders including unusual bruising, persistent fever, sore throat, or mouth ulcers, which may indicate bone marrow suppression.
  • Yellowing of the skin or eyes, which may indicate liver damage or hemolytic anemia.
  • Significant reduction in urination, which may indicate kidney involvement or crystalluria.
  • Signs of hyperkalemia including muscle weakness, irregular heartbeat, or numbness.
  • Severe allergic reaction including facial swelling, difficulty breathing, or anaphylaxis.
