What Experts Say
Pioglitazone belongs to a drug class called thiazolidinediones, sometimes shortened to TZDs. It works differently from many other diabetes tablets. Instead of pushing the pancreas to release more insulin, it makes muscle, fat, and liver cells respond better to the insulin the body already produces.
This mechanism means blood sugar drops in a steady, predictable manner rather than suddenly. Because of this action, the drug carries a lower risk of hypoglycemia when used alone compared to sulfonylureas. Full benefit on blood sugar often takes several weeks to appear since the medicine works at the cellular level.
Doctors weigh this benefit against known risks. Pioglitazone can cause fluid retention, weight gain, and in some patients worsens existing heart failure. It has also been linked to a small increase in bladder cancer risk with long term use and to reduced bone density in women. Regular monitoring of weight, swelling, and liver function is standard practice.
