How It Works
Cephradine exerts its bactericidal effect by binding to penicillin-binding proteins (PBPs) located on the bacterial cell membrane. PBPs are enzymes responsible for catalyzing the cross-linking of peptidoglycan strands that form the structural backbone of the bacterial cell wall. When Cephradine occupies these binding sites, peptidoglycan cross-linking is blocked, the cell wall loses its structural integrity, and the bacterium undergoes osmotic lysis and death. This mechanism is shared by all beta-lactam antibiotics but differs in spectrum and PBP affinity between generations.
First-Generation Limitations
As a first-generation cephalosporin, Cephradine has strong activity against gram-positive organisms including Streptococcus pyogenes, Streptococcus pneumoniae, and methicillin-susceptible Staphylococcus aureus (MSSA). Its gram-negative coverage is more limited than second and third-generation cephalosporins, with activity primarily against Escherichia coli, Klebsiella pneumoniae, and Proteus mirabilis. It has no activity against Pseudomonas aeruginosa, MRSA, Enterococcus, or anaerobes. This narrower spectrum makes it appropriate for uncomplicated community infections but insufficient for hospital-acquired or resistant infections.
Clinical Note
Cephradine resistance is rising in Pakistan due to widespread, often prescription-free antibiotic use. For recurrent or treatment-resistant infections, sensitivity testing is strongly recommended before prescribing Velosef. The full prescribed course must always be completed. Cross-reactivity with penicillin allergy affects approximately 1 to 2% of patients with a confirmed penicillin allergy history, and a full allergy assessment should be conducted before initiating therapy.

How to use Velosef Tablet 500mg?
- Velosef may be taken with or without food, though taking it with food or milk reduces the risk of stomach upset.
- The capsule should be swallowed whole with a full glass of water.
- Dairy products should be taken at least 2 hours apart from Velosef, as they may reduce absorption.
- The full prescribed course must be completed even if symptoms improve before it ends. Stopping early is a leading cause of antibiotic resistance.
- The standard adult dose for most infections is 500mg every 6 hours (four times daily) or 1g every 12 hours for 7 to 14 days, as directed by the doctor.
- The dose should not be changed without consulting a doctor.
Drug Interactions
All current medicines, supplements, and herbal products must be disclosed to the prescribing doctor before starting Velosef. Key interactions include:
- Probenecid: Reduces kidney excretion of Cephradine, raising its blood levels and prolonging its effect. This interaction is sometimes used clinically but must be supervised.
- Warfarin: Cephradine may enhance the anticoagulant effect of warfarin. INR should be monitored closely during concurrent use.
- Oral contraceptives: Cephradine may reduce the effectiveness of hormonal contraceptives by altering gut flora. Additional contraceptive precautions should be taken during and shortly after treatment.
- Other nephrotoxic medicines including aminoglycoside antibiotics and loop diuretics: Concurrent use may increase the risk of kidney damage, particularly in patients with pre-existing renal impairment.
- Metformin: Cephradine may slightly increase metformin blood levels by competing for renal tubular secretion. Blood glucose should be monitored in diabetic patients.

- Antibiotic resistance: Velosef must only be used for confirmed bacterial infections. In Pakistan, cephalosporin resistance is rising due to widespread unsupervised antibiotic use. Sensitivity testing is recommended for recurrent or treatment-resistant infections.
- Penicillin cross-reactivity: Patients with a confirmed penicillin allergy have a 1 to 2% risk of cross-reactivity with Velosef. A full allergy history must be taken before prescribing.
- difficile colitis: All antibiotics including Velosef can disrupt gut flora and trigger Clostridioides difficile overgrowth. Persistent or bloody diarrhea during or after treatment must be investigated immediately.
- Kidney disease: Cephradine is excreted primarily through the kidneys. Dose reduction is required in patients with moderate to severe renal impairment.
- Pregnancy: Velosef should only be used during pregnancy when the prescribing doctor determines the benefit clearly outweighs any risk to the fetus.
- Breastfeeding: Cephradine passes into breast milk in small amounts. A doctor should be consulted before use during breastfeeding.
- Superinfection: Prolonged use of Velosef can cause overgrowth of resistant organisms including fungi. Any new infection developing during treatment should be reported to a doctor.
- Driving and machinery: Velosef may cause dizziness in some patients. Caution is advised when driving or operating heavy machinery.

Most patients tolerate Velosef well at recommended doses. Side effects are generally mild and resolve after completing the course.
- Nausea or mild stomach discomfort.
- Diarrhea or loose stools.
- Abdominal pain or bloating.
- Headache or dizziness.

Serious side effects are rare. Medical attention should be sought immediately if any of the following occur:
- Severe or bloody diarrhea, which may indicate Clostridioides difficile colitis.
- Signs of a severe allergic reaction including facial swelling, difficulty breathing, or anaphylaxis.
- Yellowing of the skin or eyes, which may indicate liver involvement.
