What Experts Say
Tramadol works through two pathways. It binds weakly to opioid receptors in the brain and spinal cord, while also blocking reuptake of serotonin and norepinephrine. This dual action gives it a broader pain relieving effect than a typical opioid alone. Paracetamol acts mainly in the central nervous system, reducing pain signals and fever without involving opioid receptors.
Combining the two ingredients allows a lower tramadol dose to give meaningful pain relief compared to tramadol alone. This matters because opioid related side effects such as sedation and nausea tend to rise with dose. Clinicians often choose this combination for moderate pain that has not responded to paracetamol or NSAIDs alone, particularly after surgery or dental extractions.
Because tramadol is an opioid, tolerance and physical dependence can develop with regular use beyond a few weeks. Sudden discontinuation after prolonged use can trigger withdrawal symptoms. Respiratory depression risk rises when tramadol is combined with other sedating drugs or in patients with impaired liver or kidney function, so duration and dose are kept as low as necessary.

Drug Interactions
- SSRIs And SNRIs: Taking these antidepressants with tramadol raises the risk of serotonin syndrome, a potentially life threatening condition.
- MAOIs: Monoamine oxidase inhibitors should not be combined with tramadol due to a severe risk of serotonin syndrome and should be stopped at least 14 days before starting this medicine.
- Other Opioids: Combining with other opioid pain medicines increases the risk of severe sedation and respiratory depression.
- Benzodiazepines And Sedatives: Concurrent use increases the risk of profound sedation, slowed breathing, coma, and death.
- Alcohol: Alcohol increases central nervous system depression and liver strain when taken with this medicine.
- Warfarin: Tramadol can enhance the blood thinning effect of warfarin and increase bleeding risk.
- Carbamazepine: This anticonvulsant reduces tramadol blood levels and effectiveness while also raising seizure risk.
- Other Paracetamol Containing Products: Taking additional paracetamol from cold, flu, or fever medicines increases the risk of liver damage.
Risks and warnings
- Opioid Dependence: Regular use beyond a few weeks can lead to tolerance and physical dependence, so this medicine is meant for short term pain relief only.
- Respiratory Depression: Tramadol can slow breathing, especially at higher doses or when combined with alcohol, sedatives, or other opioids.
- Seizure Risk: Tramadol lowers the seizure threshold, and the risk rises in people with epilepsy, head injury, or those taking certain antidepressants.
- Liver Toxicity: The paracetamol component can damage the liver if taken above the recommended dose or combined with other paracetamol products.
- Serotonin Syndrome: Combining this medicine with antidepressants or other serotonergic drugs can raise serotonin levels to a dangerous point.
- Pregnancy And Breastfeeding: This medicine should be avoided during pregnancy and breastfeeding unless a doctor determines the benefit outweighs the risk.
