About Coralan Tablet 5mg

Coralan Tablet 5mg contains Ivabradine, a heart rate lowering agent used to start angina and heart failure treatment.

Coralan Tablet 5mg Uses

  • Used as the starting dose for chronic stable angina in adults who cannot take or do not fully respond to beta blockers.
  • Prescribed to begin treatment for chronic heart failure with reduced pumping function alongside standard heart failure medicines.
  • Used to lower a resting heart rate above 70 beats per minute in patients with a normal heart rhythm.
  • Given to reduce how often exertional chest pain occurs in stable coronary artery disease.

Coralan Tablet 5mg Benefits

  • Slows the heart rate gently at a lower dose, letting your doctor judge your response before any increase.
  • Reduces oxygen demand on the heart muscle, which can ease angina symptoms during daily activity.
  • Works alongside other angina or heart failure medicines without weakening heart muscle contraction.
  • Offers a treatment option for patients whose asthma or low blood pressure rules out a beta blocker.

What Experts Say

Cardiologists usually begin ivabradine at 5mg twice daily so they can judge how strongly a patient's sinus node responds before considering a higher dose. This starting strength slows the pacemaker current without reducing how forcefully the heart muscle contracts, suiting patients who cannot tolerate a beta blocker due to asthma or low blood pressure.

Because ivabradine only lowers heart rate in a normal sinus rhythm, doctors confirm this on an ECG before starting treatment and recheck the resting heart rate after about two weeks. If it has not fallen enough and the drug is well tolerated, the dose is raised to 7.5mg rather than adding a second agent.

Ischemic heart disease remains a leading cause of illness in Pakistan, and many patients carry conditions such as diabetes or asthma that limit beta blocker use. Physicians here often favor a low starting dose of ivabradine to control anginal symptoms safely while leaving room to adjust upward.

How to use Coralan Tablet 5mg?

  • Take Coralan Tablet 5mg by mouth twice daily with meals, ideally morning and evening.
  • Swallow the tablet whole with a glass of water, do not crush or chew it.
  • This is usually the starting dose for adults beginning treatment for chronic stable angina or chronic heart failure.
  • Your doctor will check your heart rate after about two weeks and may adjust the dose based on your response.
  • Do not stop or change the dose on your own, even if you feel better.
  • If you miss a dose, take it as soon as you remember unless it is almost time for the next one.

Drug Interactions

  • Strong CYP3A4 Inhibitors: Ketoconazole, itraconazole, clarithromycin, erythromycin, and HIV protease inhibitors raise ivabradine levels sharply and must not be combined with Coralan.
  • Verapamil and Diltiazem: These calcium channel blockers slow the heart rate on their own and also raise ivabradine levels, so this combination is not allowed.
  • Beta Blockers: Adding a beta blocker to Coralan can lower heart rate further, so your doctor will monitor you closely if both are prescribed.
  • Digoxin and Amiodarone: These heart rhythm medicines can add to the bradycardia risk when taken with Coralan.
  • CYP3A4 Inducers: Rifampicin, phenytoin, and St John's Wort can lower ivabradine levels and reduce how well the medicine works.
  • QT Prolonging Drugs: Combining Coralan with medicines that slow heart rate and also prolong the QT interval raises the risk of serious rhythm problems.

Risks and warnings

  • Bradycardia: Coralan can slow the heart rate too much, especially during the first weeks of a new treatment.
  • Atrial Fibrillation: This medicine carries a higher risk of irregular heart rhythm, so your doctor will check your pulse at follow up visits.
  • Visual Phosphenes: Some people notice brief flashes of brightness in their vision in the first two months of treatment.
  • Unstable Heart Conditions: Coralan is not safe for people with unstable angina, a recent heart attack, cardiogenic shock, or a resting heart rate below 70 beats per minute before starting.
  • CYP3A4 Interactions: Taking Coralan with strong CYP3A4 inhibitors such as ketoconazole or clarithromycin, or with verapamil and diltiazem, is not allowed since it raises ivabradine levels dangerously.
  • Pregnancy: This medicine can harm an unborn baby, so women of childbearing age must use reliable contraception while taking it.
  • Liver Impairment: People with severe liver disease should avoid Coralan since the liver clears the drug from the body.
