What Experts Say
Co-trimoxazole pairs sulfamethoxazole and trimethoprim, two agents that block sequential steps in bacterial folate synthesis. This dual blockade produces a synergistic bactericidal effect against many gram-positive and gram-negative organisms, including several strains that resist single-agent sulfonamide therapy alone.
Clinical guidelines still support this combination for uncomplicated urinary tract infections, acute otitis media, traveler's diarrhea, and Pneumocystis jirovecii pneumonia prophylaxis in immunocompromised patients. Rising resistance in some bacterial strains means local susceptibility patterns should guide first line antibiotic selection for skin and respiratory infections.
Careful patient selection remains essential for safe use. Renal function, hydration status, and any medication that raises potassium or suppresses bone marrow should be reviewed before starting therapy. Patients need clear counseling on finishing the full course, staying well hydrated, and reporting any new rash or fever promptly.
