What Experts Say
Strontium ranelate works through a dual mechanism that is different from standard antiresorptive agents. It appears to reduce osteoclast mediated bone resorption while simultaneously supporting osteoblast activity, giving it a distinct pharmacological profile among bone-active drugs.
Because of documented cardiovascular signals in large trials, prescribers now reserve this molecule for patients with no history of ischemic heart disease, uncontrolled hypertension, or peripheral arterial disease. Baseline cardiovascular risk assessment and periodic monitoring during therapy are considered standard practice.
Dosing is typically once daily at bedtime, ideally two hours after food, since calcium and food intake can reduce absorption significantly. Physicians commonly counsel patients on this timing gap to ensure adequate bioavailability, and renal function is monitored given reduced clearance in older adults with impaired kidney function.
Drug Class
- Bone-forming and antiresorptive agent

Drug Interactions
- Calcium supplements: Reduce absorption of strontium ranelate if taken close together, requiring a two hour gap.
- Antacids: Can lower drug absorption when taken at the same time.
- Tetracycline antibiotics: Absorption of these antibiotics may be reduced when combined.
- Quinolone antibiotics: Similar reduced absorption interaction, dosing should be spaced apart.
- Oral contraceptives: No significant interaction reported but concurrent cardiovascular risk factors should still be reviewed.
Risks and warnings
- Cardiovascular risk: Not recommended in patients with a history of heart attack, stroke, uncontrolled blood pressure or blood clotting disorders.
- Severe skin reactions: Rare but serious hypersensitivity reactions such as DRESS syndrome have been reported and require immediate discontinuation.
- Venous thromboembolism: Slightly higher risk of blood clots, so caution is needed in immobile or high-risk patients.
- Kidney impairment: Use with caution in patients with reduced kidney function since clearance depends on renal filtration.
- Pregnancy and breastfeeding: Not recommended as safety data in these groups is insufficient.
- Drug absorption timing: Food, milk and calcium supplements reduce absorption if taken too close to the dose.
- Elderly patients: Require closer monitoring due to age-related decline in kidney function and higher baseline cardiovascular risk.
