What Experts Say
Strontium ranelate works differently from most osteoporosis medicines. It closely resembles calcium, so the body incorporates it into new bone tissue. At the same time, it acts on bone cells in two ways, encouraging osteoblasts to build new bone while slowing down osteoclasts that break bone down.
Large clinical trials in postmenopausal women showed fewer spine and hip fractures with regular use. Later safety reviews found a higher risk of heart attack and blood clots in some patients, which led regulators in several countries to restrict its use to people without heart or circulation problems.
Doctors now reserve this medicine mainly for patients who cannot tolerate bisphosphonates or other first line osteoporosis treatments. Cardiovascular risk factors are checked before starting therapy, and heart health is monitored periodically during long term treatment.

Drug Interactions
- Tetracycline Antibiotics: Strontium can bind with tetracyclines and reduce their absorption, so a gap of at least 2 hours between doses is recommended.
- Quinolone Antibiotics: Absorption of quinolone antibiotics can be reduced if taken together with strontium ranelate.
- Calcium Supplements and Dairy Products: Calcium reduces the absorption of strontium ranelate, so these should not be taken at the same time.
- Antacids: Antacids containing calcium, magnesium or aluminum can lower the absorption of this medicine if taken too close together.
Risks and warnings
- Cardiovascular Risk: Strontium ranelate has been linked to a higher risk of heart attack, so it is not recommended for people with a history of ischemic heart disease, peripheral artery disease or uncontrolled high blood pressure.
- Blood Clot Risk: This medicine can increase the risk of venous thromboembolism, including deep vein thrombosis and pulmonary embolism, especially during periods of reduced mobility.
- Severe Skin Reactions: Rare but serious allergic reactions such as DRESS syndrome have been reported, usually within the first weeks of treatment, so stop the medicine and seek care if a rash develops.
- Kidney Function: Use with caution in patients with reduced kidney function and avoid in severe renal impairment.
- Immobilization: Temporarily stopping the medicine is advised during periods of prolonged immobility such as after surgery or extended bed rest.
- Cardiovascular Screening: A careful heart and circulation check is needed before starting therapy, especially in men and older patients with existing risk factors.
