What Experts Say
Acetazolamide works by blocking carbonic anhydrase, an enzyme involved in fluid and acid base regulation throughout the body. In the eye, this reduces production of the fluid that maintains eye pressure, which is why ophthalmologists use it to bring pressure down quickly in glaucoma, often as a short term measure alongside eye drops rather than a permanent solution on its own.
The same enzyme blocking action explains its very different use at high altitude. By encouraging mild metabolic acidosis, it speeds up the breathing pattern the body needs to adapt to lower oxygen levels, so climbers and trekkers use it preventively before ascent rather than only after symptoms start, since prevention works better than treatment once acute mountain sickness has already set in.
Physicians monitor electrolytes during longer courses, since the drug promotes loss of bicarbonate and potassium. It carries a sulfonamide structure, so a history of sulfa allergy is always checked first, and it is generally avoided in significant kidney or liver disease and adrenal insufficiency, where its effects can be harder to predict.
