What Experts Say
Levetiracetam binds to synaptic vesicle protein SV2A in the brain, a mechanism unique among antiepileptic drugs. This binding modulates neurotransmitter release and dampens the abnormal nerve firing that triggers seizures. This pathway differs from older sodium channel blocking drugs, so it is often added when other antiepileptics fail to fully control seizures.
Levetiracetam is absorbed quickly and almost completely from the gut, reaching peak blood levels within about an hour. It undergoes minimal liver metabolism and is cleared mainly through the kidneys, which keeps its interaction profile comparatively low. Its low plasma protein binding further reduces the risk of significant drug-drug interactions.
Clinically, experts position levetiracetam as an adjunctive option for partial-onset, myoclonic and generalized tonic-clonic seizures rather than a first-line monotherapy. Careful attention to mood and behavior is advised, since irritability and, rarely, suicidal thinking have been reported. Abrupt discontinuation is discouraged since it can provoke rebound seizures.
Drug Class
- Pyrrolidone derivative class of anticonvulsants
- Structurally unrelated to older antiepileptic drugs such as phenytoin or carbamazepine
- Classified as a second-generation antiepileptic drug

Drug Interactions
- Other Antiepileptic Drugs: Combining with other seizure medicines can increase drowsiness and central nervous system side effects, needing dose monitoring.
- Alcohol: Alcohol increases the drowsiness and dizziness caused by levetiracetam and should be avoided during treatment.
- Sedatives and Sleep Aids: Taking levetiracetam with sedatives, sleep medicines or opioids can deepen sedation and slow breathing.
- Methotrexate: Co-administration may raise methotrexate levels in the blood, increasing its toxicity risk.
- Hormonal Contraceptives: Unlike many older antiepileptics, levetiracetam does not significantly reduce the effectiveness of birth control pills.
Risks and warnings
- Mood and Behavior Changes: Some patients report irritability, anxiety, depression or rare suicidal thoughts; report any sudden mood change to a doctor immediately.
- Do Not Stop Suddenly: Stopping this medicine abruptly can trigger rebound seizures; the dose should be tapered under medical supervision.
- Kidney Function: Levetiracetam is cleared mainly by the kidneys, so patients with kidney disease need dose adjustment and monitoring.
- Pregnancy and Breastfeeding: Use only if the benefit outweighs the risk, and discuss seizure control options with a doctor before and during pregnancy.
- Driving and Machinery: This medicine can cause drowsiness and dizziness; avoid driving or operating heavy machinery until effects are known.
