What Experts Say
Levetiracetam works differently from older anticonvulsants. It binds to synaptic vesicle protein SV2A in the brain, which modulates neurotransmitter release and dampens the abnormal, repetitive neuronal firing that underlies seizures, rather than blocking sodium or calcium channels directly.
This distinct mechanism means levetiracetam has few interactions tied to liver enzyme induction, since it is largely cleared unchanged by the kidneys. Clinical studies support its use as add on therapy for partial onset seizures and as monotherapy in several seizure types, with dose usually titrated upward over two week intervals.
Behavioral and mood changes are a recognized class effect of anticonvulsants including levetiracetam, so clinicians monitor for irritability or low mood, especially early in treatment. Kidney function also guides dosing, since reduced renal clearance requires a lower daily dose to avoid drug accumulation.

Drug Interactions
- Other Anticonvulsants: Combined use may increase drowsiness and coordination problems, though pharmacokinetic interactions are minimal.
- Alcohol and Sedatives: These add to the drowsiness and dizziness caused by levetiracetam.
- Oral Contraceptives: Levetiracetam does not significantly reduce their effectiveness, unlike many older anticonvulsants.
- Methotrexate: Levetiracetam clearance may be reduced when given with methotrexate, raising levetiracetam levels.
Risks and warnings
- Mood and Behavior Changes: May cause irritability, agitation or low mood, and rarely thoughts of self harm.
- Sudden Withdrawal: Stopping the medicine abruptly can cause seizures to return or worsen.
- Kidney Function: Dose needs adjustment in patients with reduced kidney function, since the drug is cleared mainly by the kidneys.
- Pregnancy: Should be used only if clearly needed, under specialist supervision, as it crosses the placenta.
- Driving and Machinery: May cause drowsiness or dizziness, so caution is needed with driving until effects are known.
