What Experts Say
Hydroquinone works by blocking tyrosinase, the enzyme melanocytes rely on to convert tyrosine into melanin. When this pathway slows down, less pigment reaches the skin surface and existing dark patches gradually fade. This mechanism explains why hydroquinone remains one of the most researched and effective depigmenting agents used in dermatology today.
A four percent concentration is generally considered the standard clinically effective strength for melasma and stubborn hyperpigmentation, since lower concentrations such as two percent often produce a slower and less noticeable response. Dermatologists typically limit continuous use to a defined course, often eight to twelve weeks, before reassessing the skin and deciding whether to continue, pause, or switch to a maintenance regimen.
Prolonged or unsupervised use carries a real risk of exogenous ochronosis, a paradoxical bluish black discoloration that is difficult to reverse, along with increased photosensitivity that can worsen pigmentation if sun protection is skipped. For these reasons hydroquinone is best used under medical supervision, paired daily with broad spectrum sunscreen, and avoided during long uninterrupted stretches without a break.
