What Experts Say
Letrozole works by temporarily blocking estrogen production, which removes the negative feedback signal to the brain and prompts a rise in follicle stimulating hormone. This rise drives the ovary to develop and release a mature egg, making letrozole a practical option for ovulation induction.
Fertility specialists increasingly favor letrozole over clomiphene citrate as a first choice for women with polycystic ovary syndrome, since research shows better ovulation and pregnancy rates with fewer multiple pregnancies. Its short time in the body also means less disruption to the uterine lining, which matters for implantation.
Infertility linked to polycystic ovary syndrome is common among women of reproductive age in Pakistan, and letrozole based protocols are now a standard first step before considering injectable stimulation or assisted reproduction. Doctors typically monitor follicle growth with ultrasound to time intercourse or insemination accurately.

Drug Interactions
- Tamoxifen: Concurrent use may lower letrozole levels and is generally avoided in combined protocols
- Estrogen Containing Medicines: Hormonal supplements or estrogen therapy can work against letrozole's action and are avoided during treatment cycles
- Gonadotropins: Sometimes combined under specialist supervision for controlled ovarian stimulation, requiring close monitoring for over-response
- CYP450 Interacting Drugs: Strong CYP3A4 or CYP2A6 inducers and inhibitors could theoretically alter letrozole levels though significant interactions are uncommon
Risks and warnings
- Pregnancy Risk: Letrozole must be stopped once pregnancy is confirmed since it can harm fetal development
- Multiple Pregnancy: Ovulation induction slightly raises the chance of twins compared to a natural cycle
- Ovarian Cysts: Functional ovarian cysts can form during treatment and are usually monitored by ultrasound
- Liver Impairment: Use cautiously in patients with liver disease since drug clearance may be reduced
- Bone Health: Extended or repeated use over many treatment cycles may affect bone density over time
