What Experts Say
Letrozole works by blocking the aromatase enzyme, which converts androgens into estrogen in fat, muscle and adrenal tissue. In postmenopausal women, this peripheral pathway becomes the main estrogen source once the ovaries stop functioning, making aromatase inhibition a more direct target than blocking estrogen receptors alone.
Oncologists often prefer letrozole over tamoxifen for postmenopausal women with hormone receptor positive tumors, since it lowers circulating estrogen more completely and shows a lower recurrence rate in several large trials. Regular bone density scans and cholesterol checks are advised, since estrogen suppression can accelerate bone loss and shift lipid levels over years of continuous treatment.
Breast cancer remains the most commonly diagnosed cancer among women in Pakistan, and many cases present at a hormone receptor positive stage where letrozole based therapy fits well. Physicians stress strict daily adherence for the full treatment course, since inconsistent use meaningfully raises the chance of disease recurrence.

Drug Interactions
- Tamoxifen: Combining with tamoxifen may lower letrozole blood levels and reduce its effectiveness, avoid concurrent use
- Estrogen Containing Medicines: Hormonal contraceptives or hormone replacement therapy can counteract letrozole's estrogen lowering effect
- Bisphosphonates: Often co-prescribed to protect bone density, requires coordinated monitoring rather than causing harm
- CYP450 Interacting Drugs: Strong CYP3A4 or CYP2A6 inducers and inhibitors could theoretically alter letrozole levels though significant interactions are uncommon
Risks and warnings
- Pregnancy Risk: Letrozole can cause fetal harm and must not be used during pregnancy
- Bone Health: Long term use lowers bone density and raises the risk of osteoporosis and fractures
- Liver Impairment: Use cautiously in patients with liver disease since drug clearance may be reduced
- Cholesterol Changes: May raise blood cholesterol levels, requiring periodic monitoring
- Premenopausal Use: Not intended for premenopausal women since the ovaries still produce estrogen
- Driving and Machinery: May cause dizziness or fatigue that affects the ability to drive safely
