What Experts Say
The 500mg strength is usually reserved for adults who need higher weight based doses or extended interval dosing, a strategy that can lower toxicity while keeping the drug effective against resistant gram negative bacteria. Specialists choose amikacin when first line antibiotics have failed or local resistance patterns favor an aminoglycoside.
Because higher doses raise the risk of kidney and hearing damage, doctors check renal function, fluid status and hearing regularly during a course of amikacin. Trough levels are watched closely in critically ill patients, and the dose is adjusted if kidney function changes during treatment.
In intensive care and resistant infection cases, amikacin is often combined with a beta lactam antibiotic to broaden coverage while cultures are pending. This injection needs close medical supervision, and therapy is stopped or switched once sensitivity results are known to limit resistance in Pakistan.
