What Experts Say
Doctors across Pakistan often choose nimesulide for short courses of pain relief because its preferential COX-2 selectivity produces less gastric irritation than many older NSAIDs, though this pharmacological advantage does not remove the need for strict duration limits during treatment planning.
Physicians typically recommend baseline liver function testing before starting therapy in patients with any history of hepatic disease, since nimesulide carries a well documented hepatotoxicity signal that becomes clinically more relevant once treatment extends beyond ten to fifteen days of continuous use.
Clinicians generally reserve nimesulide for acute musculoskeletal, dental and post-operative pain in otherwise healthy adults, favouring paracetamol or topical anti-inflammatory options first in elderly patients or those managing several coexisting chronic illnesses together.

Drug Interactions
- Warfarin: Nimesulide can increase the blood-thinning effect of warfarin, raising the risk of bleeding.
- Other NSAIDs: Combining with other NSAIDs raises the risk of stomach ulcers and kidney strain.
- Methotrexate: May increase methotrexate blood levels and toxicity risk.
- ACE Inhibitors and ARBs: Can reduce the blood-pressure-lowering effect and raise the risk of kidney problems when combined.
- Lithium: May raise lithium blood levels, increasing the risk of toxicity.
- Diuretics: Can reduce the effectiveness of diuretics and increase the risk of kidney impairment.
Risks and warnings
- Liver Risk: Nimesulide carries a recognised risk of liver injury, so treatment courses are usually limited to fifteen days or less.
- Pregnancy: Not recommended during pregnancy, especially in the third trimester, due to risks to the unborn baby.
- Breastfeeding: Avoid use while breastfeeding unless a doctor has confirmed it is necessary.
- Kidney Impairment: Use with caution in patients with reduced kidney function, as NSAIDs can further reduce renal blood flow.
- Peptic Ulcer: Contraindicated in patients with active or a history of peptic ulcer disease or gastrointestinal bleeding.
- Elderly Patients: Older adults are more prone to side effects and should use the lowest effective dose for the shortest time.
- Alcohol: Avoid alcohol during treatment as it increases the risk of stomach irritation and liver strain.
